Evaluation of the Norwegian Breast Cancer Screening Program

To provide a robust and comprehensive evaluation several complex methodological issues will be thoroughly addressed and discussed: 1. The low statistical power due to the small size of the Norwegian population. 2. The influence of factors other than scree ning on breast cancer incidence and mortality trends, including the dramatic changes in hormone therapy use among postmenopausal women and changes in the treatment of breast cancer during the implementation of the organised program, 3. The extensive use o f opportunistic mammography screening even before implementation of the organised program. 4. The complexity of the NBCSP data base. 5. The possible diagnostic and therapeutic differences between screen-detected and non screen-detected breast cancer pati ents. To secure effective and high quality input for this evaluation, we have organized the work with three levels of responsibility. First, a Norwegian principal investigator and a Norwegian scientific project manager with in-depth knowledge on health care in Norway will be responsible for the day-to-day work. Second, an international Active Research Group of epidemiologists and statistician with broad and long term experience in screening and breast cancer research will provide regular imput into the work. Third, a Norwegian Advisory Board of breast cancer specialists, working in breast cancer research, diagnosis and treatment will provide the necessary input on clinical aspects and information about the NBCSP. The Advisory Board will include speciali sts in molecular genetics, ethics and social science. We think this combination of Norwegian project leadership, an international Active Research Group of experienced epidemiologists/statisticians, and a Norwegian Advisory Board of researchers and clinici ans will be the very best combination for a high quality and unbiased evaluation of the NBCSP.
