SmartScan - Ultralyd for "Dummies"

The SmartScan project has focused on research and development of technological solutions which will help make handheld ultrasound useful to medical personnel without in-depth training in ultrasound scanning. The main projects dealt with augmented reality (AR), automation, protocol-based procedures, as well as mobile communication solutions. In the AR-project a prototype running on iPads was developed. The system automatically finds known landmarks in the ultrasound images and connects these to the landmarks of a digital heart model in a manner which allows the user to see, in real time, exactly where in the model these section images have been taken. The method has been refined by using a camera to superimpose the ultrasound image with the heart model on the patient. The result is almost like having "X-ray vision." If the clinical evaluation with medical students planned spring 2017 is successful, the method is likely to be realized in a commercial product. Researchers at NTNU has developed an image based method for automatic measurement of cardiac function. The algorithm finds the heart and left ventricle automatically and measures contractility/ displacement (MAE). Clinical trials with 200 patients and 167 healthy volunteers has been conducted, and with a 10 mm cut off, the classification of cardiac function (MAE) is correct in 90% of the cases, fully automatic. This is very promising for introduction of ultrasound of the heart by non-experts. In cooperation with NTNU, Fimreite Software has developed and evaluated a novel solution for transferring images from an ultrasound device operated by a nurse in an outpatient clinic, to a cardiologist at the hospital. The purpose is to demonstrate the usefulness of the latest cloud solution in treatment and follow-up of chronic heart failure patients. Data collection is complete and analysis ongoing. The results will be published in 2017 and if positive the method will be commercialized. The SmartScan project ended in Dec 2016 and has resulted in several technical solutions and prototypes, which the partners GE Vingmed Ultrasound AS and Fimreite Software AS will be implementing and integrating in their own product launches. The research has contributed 20 scientific papers and two book chapters so far.

GE Vingmeds håndholdte ultralydscanner "Vscan" har siden lanseringen for to år siden fått meget god respons blant kardiologer og andre spesialister med erfaring i ultralyd-diagnostikk, som et nyttig supplement til den kliniske undersøkelsen og tradisjonel l ultralydundersøkelse. I dag er det i Norge en liten andel fastleger som bruker ultralyd på legekontoret, så det vil medføre en betydelig innovasjon og endret praksis for helsevesenet, hvor målet er å kunne gi pasientene en bedre diagnostikk og oppfølgi ng i primærhelsetjenesten og redusere behovet for ekspertvurderinger i sykehusene, og slik også være av betydning for Samhandlingsreformen. Den største barrieren for introduksjon av ultralyd og lommeultralyd i primærhelsetjenesten er mangel på kompetanse hos ikke-spesialistene, både når det gjelder å gjennomføre avbildning av høy kvalitet, og i tolkning av bildefunn. Det forventes at systematisk opplæring for eksempel i medisinstudiet vil øke bruken av håndholdt ultralyd i årene fremover, men det vil san nsynligvis ta lang tid før denne teknologien tas i bruk av det store flertall av helsearbeidere i førstelinjetjenesten. I dette prosjektet vil samarbeidspartnerne fokusere på forskning og utvikling av teknologiske løsninger som skal gjøre håndholdt ultra lyd tilgjengelig uten omfattende forutgående opplæring (derav tittelen "Ultralyd for dummies"). Dette er særlig aktuelt i førstelinjetjenesten, hvor regelmessig bruk og trening i ultralydundersøkelser blant personellet er lite realistisk, eller for ressu rskrevende. Dette skal løses ved å gi brukeren audio-visuelle instruksjoner, automatiske målinger av relevante fysiologiske parametre og forslag til tolkninger (computer aided diagnostics). Mobile kommunikasjonsløsninger med "sky"-teknologi vil benyttes for effektiv lagring og kommunikasjon med spesialist.
