This application refers to results of the research group working on the ongoing project: Chronic disease management-implementation and coordination of health care systems for depressed elderly persons' project funded by The Norwegian Research Council in 2 010 (No. 204238/V50, 2011-2014). We apply for funding to disseminate the results by means of a conference and workshop. During 2011 and 2012, the research group had a productive phase comprising data collection, implementation, analysis and publication of results. The work of the group focused on establishing the evidence base, identifying and developing theory and modelling processes as well as outcomes for the target group, i.e., persons suffering from chronic disease.
An outcome-focused conference will be arranged on the means of and methods available for creating practical utility from the results of the project. The conference will build on the evidence and knowledge gained from the research process and will provide examples of potential utilization scenarios. The above-mentioned parts of the project and the research questions will be addressed and answered based on evidence developed during the project period. In addition to the research group, five leading international researchers in the field wil l be invited to contribute to the conference.
The following issues will be addressed: The chronic care model for the management of depression; The potential of implementation research to achieve better mental health; Examples of implementation research fo cusing on effective leadership and its impact on patient care in the community;
Implementation research: what it is and how it is defined; and
Patients' perspective and their experiences of family support and the health care services. The research questi ons will be addressed and answered based on evidence developed during the project period. Five leading international researchers in the field will be invited to contribute to the conference and workshop.
