The HEPI (Health Economics for Policy Impact) proposal utilise an existing European research infrastructure, the ELIXIR, to develop the scientific integration of Europe and Africa in the area of health economics, and to promote better integration of research in policy processes. Our Objectives for HEPI are directly aligned with the key areas of ‘sustainable growth and jobs’ and ‘peace, security and governance’ outlined in the European Union and European External Action Service’s ‘Towards a comprehensive strategy with Africa’, specifically in the field of population health and health system strengthening. We will (i) facilitate improved policy and research interactions in health economics and financing in Africa, (ii) foster alignment between academic and political infrastructures in health economics across Europe and Africa and facilitate the sharing of resources, (iii) strengthen capacity at institutional level, and (iv) strengthen the capability of individual researchers and policymakers to produce, interpret and apply health economic evidence.   We will establish and expand Health Economics Communities of Practice (HE COPs) in both East and West Africa. These platforms will bring together stakeholders from member states’ policy and academic communities to strengthen health economics knowledge, support the uptake of research evidence into policy, and enhance collaboration on pertinent health policy issues. We will align academic and political infrastructures in Europe and Africa in the field of health economics, and lay the foundation for sharing of materials, methods and data. We will support higher education institutions to enhance their capacity in training highly skilled professionals proficient in health economics methods. Finally, we will promote individual capability building by providing personalized guidance and support through a 2 year mentorship program. 

Health Economics for Policy Impact

This SUSCARE project addresses the call for proposals on global health by proposing an international partnership between Norwegian and Colombian Universities, Research Institutes, and International Diabetes Federation, Colombia. The primary objective of SUSCARE is to establish a long-term partnership on advancing “SDG 3-Good Health and Well-being” by fostering the Norwegian government`s “Better Health, Better Lives” goal. The main purpose of this pre-project is to build a consortium and lay the theoretical and practical foundations for applying for Collaborative and Knowledge-building Projects (KSP) in Global Health in 2025 aimed to implement and evaluate a sustainable and equitable Artificial Intelligence of Things (AIoT) Based Patient-centred framework for self-management of type 2 diabetic (T2D). Overall, this pre-project will help to get a better overview of the research challenges, perform necessary assessments of user requirements and research needs to improve self-management of T2D, identify, and engage potential stakeholders, and make concrete development goals and research direction for the KSP application to be submitted in March 2025.

A Sustainable AIoT Based Nutritional Recommender for Self-Management of Type 2 Diabetes among Adult Population

The partners of the present pre-project want to enhance knowledge and build capacity in youth health across all three partner institutions. This will be done by a) disseminating of updated knowledge on adolescent's physical, mental, and social health through 2 PhD projects based on data collected in the population-based Lusaka Fit Futures Study in Zambia in 2024, b) facilitating research through 2 PhD projects and promote high-quality services in response to injuries through a Spinal Cord Injury Registry at KCMC, which is a referral hospital with a specialized unit for traumatic spinal cord injuries in Tanzania c) establishment of MSc programs in physiotherapy at the south partner universities in Zambia and in Tanzania. The north partner's research group “Public Health and Rehabilitation” (FFR) holds expertise in population-based studies, patient’s registries and training of physiotherapists at MSc level and will support the research activities and supervision of the project’s PhD students. 
Lusaka Fit Futures Study and the Spinal Cord Injury Registry represent classical epidemiological research. To enhance the main projects' ability to provide new knowledge directly to stakeholders, bridging epidemiology with implementation, public involvement through strong end-user groups are needed. Relevant partners are formal representatives from the four participating governmental secondary schools in the Lusaka Fit Futures Study where the general adolescent population attend. In conjunction to the Spinal Cord Registry, NGOs such as the Atlas Alliance or other associations supporting young disabled people offer opportunities for important end-user groups. Funding for this pre-project will be used to identify and establish local partnerships outside the research sector in Lusaka in Zambia and in the Kilimanjaro and Arusha Regions of Tanzania, including involvement of the respective physiotherapy associations.

An international collaboration on adolescent's health situation in Zambia, Tanzania and Norway.

This pre-project aims to create a sustainable alliance of partners, both inside and outside academia, to focus on primordial prevention of cardiovascular disease in adolescents in Nepal. According to the World Health Organization, cardiovascular diseases (CVDs) are the leading cause of disability and death globally. In 2019, 32% of all global deaths were caused by CVDs, with more than three quarters of these in low- and middle-income countries. Most CVDs can be prevented by addressing risk factors connected to behaviour and the environment which also necessitates a life course approach as well as multisectoral collaboration. To intervene as early as possible is therefore crucial. Primordial prevention is particularly promising in resource-limited settings such as in a low-income country like Nepal where around 98% of the population have at least one risk factor for CVD.

To be able to create and establish a sustainable alliance of partners from Norway, Sweden, and Nepal, we require time and active engagement from partners in various sectors. Our ongoing research has provided a network of partners in Nepal within academia, such as Kathmandu Medical College and Patan Academy of Health Sciences. The network will now be expanded with several partners in Nepal that represent a broader geographic area and demographic profile than only the Kathmandu Valley as well as new partners outside academia such as the Nepal Non-Communicable Disease (NCD) Alliance. We will also engage additional partners as required from Norway and Sweden with the aim to establish the optimal consortium to tackle the identified health challenge.

The pre-project has activities planned for 3.5 months. Throughout this time, regular meetings will be held online as needed. Both UiT and our main partner institutions in Nepal will be involved in identifying partners. In addition, UiT will lead the engagement of stakeholders while Nepali partners will be responsible for local coordination on site.

Creating a sustainable partnership to effectively address primordial prevention of cardiovascular disease in adolescents in Nepal

Mental disorders are among the leading public health challenges, with widespread impacts on individuals, families, and communities, especially in low-resource settings. Vulnerable populations, including internally displaced persons, returnee migrants, individuals living in poverty, and disadvantaged communities, are at an increased risk of mental disorders. Despite the high burden, only a small fraction of affected individuals receive adequate care in low—and middle-income countries. This project addresses significant gaps in access to treatment and the need for locally relevant solutions to reduce stigma, support recovery, and empower communities.
This project aims to establish a strong, collaborative partnership to create sustainable, community-based interventions for mental disorders in Ethiopia and South Africa. Through building partnerships among research institutions, NGOs, and health authorities, we will develop scalable, culturally adapted interventions that align with SDG 3, "Good Health and Well-Being."
The primary goal is to lay the foundation for a comprehensive project proposal by developing evidence-based interventions and enhancing local research capacity. Key activities include workshops, research stays, and strategic meetings to define user needs and research qu and develop a grant proposal. The focus is on equitable partnerships, ensuring shared leadership and resource distribution.
This partnership will improve health outcomes by addressing mental health treatment gaps with digital solutions and peer support networks. The project will strengthen healthcare capacity, reduce stigma, and promote sustainable, community-driven recovery models. In the long term, it aims to create scalable, locally adapted treatment models and contribute data to guide policy, aligning with SDG 3.

Building Sustainable Partnerships for Community-based Mental Health Interventions in Ethiopia and South Africa

Lead exposure is a critical global health issue impacting about one-third of children globally, with Southeast Asia with Southeast Asia facing the greatest burden. Despite this, research in many low- and middle-income countries, including Vietnam, is sparse. In May 2024, UNICEF and the US Agency for International Development urged actions to eradicate childhood lead exposure, a goal supported by the Vietnamese government. However, existing research in Vietnam is mostly confined to small-scale studies near lead-recycling facilities or mines, showing elevated blood lead levels in children. Crucially, there is a lack of comprehensive data on lead exposure among the general population, particularly in children and pregnant women, which leaves major exposure sources and hotspots unidentified. 

For the past decade, UiT The Arctic University of Norway has developed equitable partnerships with Georgian institutions, focusing on maternal and child health and lead poisoning prevention. Fifteen years ago, UiT also collaborated with Vietnamese institutions on environmental health and epidemiology. Given the ongoing lead exposure challenges in Southeast Asia, Vietnam's renewed commitment to address this issue, and the interest from past partners to re-engage, the upcoming Research Council of Norway call in March 2025 is a perfect opportunity to enhance and extend our collaborations, including also non-research entities. Developing strong, equitable partnerships and a competitive proposal requires time and both virtual and in-person interactions. This pre-project aims to secure resources for these activities, planning online meetings to renew and expand partnerships, a 5-day workshop to refine research questions, engage stakeholders, and distribute responsibilities fairly. This will be followed by biweekly online meetings to collaboratively draft the research proposal for submission, ensuring all partners have significant input and joint ownership of the project.

Establishing sustainable partnerships for preventing childhood lead poisoning in Viet Nam

Worldwide, nearly 2 million children are living with HIV, with 160,000 new infections annually. Untreated, HIV progresses to AIDS, which kills 100,000 children annually, mainly in African countries (UNAIDS 2022). These deaths are now largely preventable due to the widespread availability of free antiretroviral therapy (ART). ART is a highly effective treatment, allowing HIV patients of all ages to live near-normal lives. ART also prevents HIV transmission during childbirth and breastfeeding. Yet, lapses in treatment, for both mothers and children, lead to many unnecessary new infections and deaths. 

There is an urgent need to understand the needs of HIV-affected children and their families, and to identify practical approaches to prevent paediatric infection, promote adherence to treatment, and empower families with the knowledge that HIV-positive children can lead long, healthy and productive lives, if they get the right care.

In order to address these knowledge needs, we must build a strong and equal partnership that can effectively identify the distinct barriers to HIV care for children, and co-design effective interventions. This collaboration must include an interdisciplinary team of health researchers with local and contextual knowledge, as well as public sector healthcare providers and local health organizations.

We will use the pre-project to travel to Malawi and meet with potential partners. The purpose of these meetings will be to better understand the needs of children affected by HIV, strategic considerations of policy actors, and the scope for the establishment of an equal and effective partnership across a network of collaborators. This will allow us to develop theories and potential interventions that effectively address research and policy needs.

Investments, Goals and Outcomes for Children Living with HIV (IGOHI)

The Philippines is frequently facing many extreme weather events that seriously affect mental health. The Mind4Climate pre-project aims to address the growing mental health crisis in the country associated with climate change. This project seeks to establish a partnership among researchers, healthcare providers, NGOs, and government agencies from Norway and the Philippines. We plan to conduct a comprehensive needs assessment, followed by designing community-based interventions and workshops that build local capacity to collaborate effectively. We, therefore, plan to design a culturally relevant, scalable project involving diverse experts from health economics, mental health, and global health ethics expertise. Depending on the foundation of this pre-project, we seek to effectively address climate-induced mental health challenges and build a research proposal for the effects of climate change on vulnerable communities in the Philippines.

Climate Change Impact on Mental Health: The Philippines Case

The AfroAccessAbility project addresses critical health disparities experienced by disabled persons in Ghana and Tanzania, with the overarching goal of improving health outcomes for disabled persons in the Global South. Despite some advancements, disabled persons face significant barriers to healthcare access that include communication challenges, discriminatory attitudes and practices, which disproportionately impact women with disabilities. The project aligns with SDG 3 ("Good health for all") and SDG 10 (reduced inequalities), focusing on bridging the health gap between disabled and non-disabled persons.

The pre-project phase focuses on building a strong consortium through co-creation with African partners. This ensures the project design integrates the lived experiences of disabled persons with rigorous academic insights, creating a robust foundation for the main project. Additionally, the pre-project aims to strengthen research collaboration networks among Organization of Persons with Disabilities (OPDs), academic institutions, policymakers, and practitioners, fostering interdisciplinary partnerships and long-term impact.

Key challenges include addressing the lack of comprehensive data on health disparities between disabled and non-disabled persons, exploring variations within disability groups, and developing practical, culturally appropriate strategies to overcome barriers. Particular emphasis will be placed on understanding relevant aspects, such as discriminatory practices, insufficient provider training, and inadequate communication strategies. These challenges will be addressed using innovative frameworks, such as positive deviance approaches, tailored to the local contexts of Ghana and Tanzania.

By bridging community insights and academic knowledge, AfroAccessAbility aims to advance inclusive health systems, reduce health inequities, and promote better health outcomes for disabled persons in the Global South, with scalable relevance for other regions.

Equality in Health Access for Disabled Persons

Contested EU Foreign Policy in an Era of Geopolitics

The doctoral network EUFOG will contribute to a better understanding of the ways in which the EU is reconsidering key tenets of its international role in the face of the geopolitical turn in international politics. The liberal international order, i.e. the collection of norms, institutions and power relationships that have defined the last decades of international political and economic relations, is undergoing major transformations. Although the final destination of these changes is still to be seen, the situation is shaped by a return of competition between great powers in a multipolar world (US, China, EU and Russia), facilitated further by growing geopolitical ambitions of many regional powers.  These developments should lead to a systematic overhaul of research about the international role of the EU. Over the past decades, the EU’s role in international politics was perceived through the prism of two assumptions. First, the external relations of the EU were understood as reflecting the kind of polity the EU was: an integration-through-law project, and a community of values. Support for multilateralism and the promotion of certain international norms were seen as the unproblematic externalization of internal consensuses. Second, even when the EU entertained projects of reform for the international order, they sought to strengthen its institutions and norms, in a moment when such strengthening was perceived as being broadly in line with the trajectory of international politics. This state of affairs has ceased to exist. EUFOG will train a generation la scholars to enable them to address the politics (the political conflicts and debates), policies (decisions and measures) and partners (relationships and perceptions) associated with the ways in which EU foreign policy responds to these new international realities in a broad range of issue areas, from security to trade to human rights.

Dronetransport av biologiske prøver lanseres som fremtidig løsning i helsetjenester. Tåler blodprøver turbulens og vibrasjon under dronetransport? Mens tidligere studier av blodprøver under dronetransport har vært utført på normale blodprøver transportert under rolige værforhold, gjennomførte vi den første studien som er gjort på effekten av turbulens på blodprøver fra pasienter. Vi simulerte 1- og 2-timers drone-flygninger med turbulens fra 10-30 G. De fleste analysene tålte dette, men for prøver som var i plasmaglass ble flere prøveresultater sterkt endret og med økende endringer fra 10 G til 30 G. Vi antar dette skyldes at slike prøver sentrifugeres og at blodlegemer som separeres fra serum blir ødelagt av ristingen som turbulens over lang tid forårsaker. Da vi testet blodprøvene under turbulens uten sentrifugering, var alle analysene normale. 
Konklusjon: Forbehandling av blodprøver før drone flygninger er viktig, og sentrifugering av plasmaprøver ikke bør gjøres. Man må finne løsninger for hvordan plasmaprøver skal tolerere flyvninger med mye turbulens. 

VINDANALYSER FOR DRONE-TRANSPORT

Anvendelse av droner i helsetjenester øker og får økende bruksmuligheter; leveranse av medisiner og vaksiner, blodprøvetransport og leveranse av utstyr i nødssituasjoner. Kraftig vind kan være en vesentlig faktor som truer stabiliteten i slik transport, og værmønstre i mikroskala kan bli viktig.
Vårt prosjekt har utviklet en metodikk for å beskrive vindforhold i urbane landskap. Metoden innebærer flerskala analyser med kobling av tre modeller med forskjellige skalaer, en numerisk operativ meso-skala værprognosemodell, en mikroskalamodell for terrengindusert vindpåvirkning, og en supermikroskala for å fange bygningsindusert vindpåvirkning. Stor variasjon i utbredelsen av atmosfæriske strømninger nødvendiggjør en kobling mellom forskjellige modeller som hver håndterer et bestemt skala-område. Artikkelen beskriver hvordan en slik flerskala-metode med kvalitative sammenligninger av vindhastigheter kan forutsies fra forskjellige numeriske modeller og målte reelle eksperimentdata og drøfter verktøyets potensial for droneoperasjoner.



A CONCEPTUAL APPROACH TO TIME SAVINGS AND COST COMPETITIVENESS ASSESSMENTS FOR DRONE TRANSPORT OF BIOLOGIC SAMPLES WITH UNMANNED AERIAL SYSTEMS (DRONES): HVORDAN SKAL VI BEDØMME TIDSBESPARELSER OG AKSEPTABLE KOSTNADER FOR DRONERANSPORT AV BLODPRØVER?
Argumentene for at dronetransport kan spare transporttid og forbedre servicepåliteligheten for transport av biologiske prøver i helsetjenesten er hovedsakelig at droner ikke hindres av trafikk og kø på veiene. Ingen studier har evaluert hvilken tidsbesparelse som er mulig og kostnadene ved droneløsninger er ukjent, de fleste studier sammenligner kjøre/flytiden for biler og drone, og ingen har vurdert hva man er villig til å betale for en gitt tidsgevinst. Det avgjørende målet i klinisk perspektiv er tiden fra prøver blir tatt til de er ferdig analysert. Det er heller ingen studier som har vurdert hva en gitt tidsgevinst er verdt. Hva er man villig til å betale for en time spart transporttid av rutineprøver? Har det noen klinisk betydning?
	Denne studien brukte total transporttid målt fra en prøve er klar for transport med bil eller drone til ankomst til laboratoriet der den skal analyseres. Et eksempel: Ved en enkelt transport på slutten av en dag i et legesenter som tar prøver jevnt gjennom dagen, vil prøvene tatt tidlig på dagen ha ventet 7-8 timer før de fraktes. Snitt ventetid for alle prøvene i en last før transport vil da være ca. 4 timer. Dersom biltransport tar to timer, vil snitt total transporttid være 6 timer med bil, med en drone som tar 1 time, vil total transporttid med drone være 5 timer, altså en time spart = 17% forkortet transport tid. Ved to transporter med 4 timers mellomrom er tilsvarende transporttider 4 timer for bil og 3 timer for drone, altså er besparelsen økt til 25 %. Men antall prøver som transporteres vil være halvert i hver transport, dvs. kostnaden er doblet. 
	Med avstander på 4.7 km (bymodell) og 262 km (distriktsruter) var tidsbesparelsen mindre enn 20 %-30 % i bymodellen men 65 %-74 % i distriktsruter på betingelse av dronehastigheter over 100 km/t, avhengig av antall transporter.  
	Studien viser at droneløsninger gir marginale gevinster for korte transporter (inner-city model), mens tidsbesparelser er mer lovende i lange transportmodeller med hensiktsmessig planlegging og tilstrekkelig høye dronehastigheter.
Konklusjon: Regulære ruter for transport av blodprøver kan ha tvilsom verdi, men kombinasjon av transport for forskjellige formål som biologisk materiale, hjemtransport av medisinsk utstyr/medisiner til pasienter/til distrikt kan skape bærekraftige droneløsninger.

The project will lay the foundation for an implementation of a fast, secure and reliable 24/7/365 UAV based medical transport service between the OUS hospitals.

OUS cover four of the five largest hospitals in Norway, all in the capital of Norway, with a current annual patient activity of 94,000 hospitalizations, 45,000 day-care treatments and 853,000 outpatient consultations and with 24,000 employees. OUS represents a highly relevant institution for studies of both the medical, organisational, economical and technical perspectives of Unmanned Aircraft Systems (UAS) services for medical purposes. Being a national hospital offering services varying from local hospital services to transplantations and advanced national services, OUS represents a complexity that ensures a wide perspective of dissemination and applicability of the results nationally and internationally.

The current mission critical transportation services between the hospitals depends on road based transport with dedicated vehicles and taxies resulting in a service that is slow, has varying capacity and with unpredictable transport time due to big city congestion. The transportation service also represents a risk due to the lack of control and status of the transports in process.

This project will address the two issues highly relevant to Norway, and not being researched in other known international UAS projects, (1) weather related challenges in a congested city with high variations in weather and (2) hospital processes and health economy scenarios.

The participants in the project represent the best competencies in Norway with strong international connections: Oslo University Hospital, Norwegian Defence Research Establishment, The Norwegian Meteorological Institute and Sintef.

Research an implementation of Unmanned Aircraft Systems for a fast and secure transport of biological material and blood products

Project objectives: Personalized medicine enabled by big data in a cognitive computing approach will be studied and clinically verified in  four groups of patients: Monogenic disorders, sudden cardiac death, colon cancer with liver metastasis and lung cancer. The project will identify cancer patients who will receive actionable and targeted molecular treatments prolonging life expectancy and improving quality of life for large groups of patients. This will translate into lowered diagnostic and treatment health care costs for the society and higher success rate for large patient groups. 
For patients with monogenic disorders the implementation of genomics and associated IT decision support tools will help eliminate the "diagnostic odyssey" characteristic of today´s practices. An accurate genetic diagnosis will help identify possibilities for preventive health care and in all cases lead to more precise prognosis.

The opportunity to provide life-saving therapy for cardiac disease is enabled by the combination of genomics, electronic health records, imaging, clinical data and Norwegian registries. 
The project will involve leading international research institutions and enterprises that will have a long-term positive impact beyond this specific project.
  
R&D challenges: As an ever-larger amount of data from multiple sources and in multiple formats are digitized and travels across organizational boundaries, there is a set of complex technology, policy and legal issues that need to be solved in order to manage big data. In healthcare, these issues are further aggravated by privacy and liability concerns, and by the need of clinicians to trust the information displayed in integrated information systems. Personalized and cost-effective health care through innovative use of big data and cognitive computing

Verdens Helseorganisasjon anbefaler å forbedre mødrehelsetjenesten ved å implementere modeller for jordmor-ledet kontinuitet i omsorgen, og å tilpasse slike modeller til lavinntektsland med adekvat jordmorutdanning. Forskning har vist omfattende fordeler med slike modeller og mer forskning på dette området er etterlyst. 
Det palestinske helsedepartementet lanserte i 2013 en modell med jordmor-ledet kontinuitet i omsorgen for landsbykvinner på den okkuperte Vestbredden. Implementeringen ble utført i samarbeid med Norwegian Aid Committee (NORWAC), finansiert av det norske utenriksdepartementet. 
Jordmødre ansatt i palestinsk offentlig helsesystem var få og hadde stort arbeidspress på sykehusene og begrenset autonomi og mulighet til å jobbe med svangerskapsomsorg. Kvinner på landsbygda hadde dårligere helsetilbud enn i byene og væpnede israelske soldater og settlere hindret kvinnenes bevegelsesfrihet. En innovativ organisering av jordmor-ressursene gjorde forbedringer mulig. Denne studien undersøker effekten av å implementere en jordmor-ledet modell for kontinuitet i omsorgen, der jordmødre fra de offentlige sykehusene kjører ut til faste avsidesliggende landsbyer ukentlig. Jordmødrene følger opp hver sin faste gruppe kvinner gjennom svangerskapet og ved hjemmebesøk etter fødsel. Jordmødrene har ansvar for oppfølging av kvinner i hver sin landsby, som de besøker hver uke. Målet var å forbedre kvinnenes tilgang til og benyttelse av et godt helsetilbud som bidrar til bedre helse og tilfredshet.
De tre kvantitative observasjonsstudiene gir avgjørende bakgrunn for en helhetlig vurdering av implementeringen av jordmor-modellen. Den første, et klynge-studie, fant at kvinnene som fikk jordmor-modellen, sammenlignet med de som ikke fikk, benytter helsetilbudet oftere, at flere fikk avdekket komplikasjoner i svangerskapet som førte til henvisning, og at flere kvinner fikk forbedret barselomsorg, med tettere oppfølging av mor og barn, blant annet ved hjemmebesøk. Den andre studien, en kohort, fant at færre kvinner som fikk jordmor-modellen, enn de som ikke fikk, hadde akutte keisersnitt, induksjon av fødsel, anemi etter fødsel og blodtransfusjon, de fikk færre premature og færre syke barn og barn med veldig lav fødselsvekt. Den siste, en case-kontroll studie, påviste at kvinner som fikk jordmor-modellen var mer tilfredshet med omsorgen gjennom både svangerskap, fødsel og barseltid og at flere fullammet lenger.
Forskningen har fått fram ny kunnskap om jordmor-ledet kontinuitet i omsorgen i et lav-middels inntektsland under militær okkupasjon. Studiene legger til og støtter opp om resultater fra tidligere forskning på slike modeller i høyinntekstland. Resultatene bekrefter anbefalingen fra WHO om å implementere jordmor-ledete modeller for kontinuitet i omsorgen tilpasset til lavinntektsland.

The main objective  is to validate a model of Continuity of Midwifery Care that is under implementation in the Palestinian governmental hospitals. The implementation of the model started in 2012 as a Norwegian funded project, aiming to improve reproductive health-services and equity for poor women in remote areas in the occupied Palestinian Territories (oPT) defined by OECD as a Low-middle income territory.

The workforce of qualified midwives in Palestine is low, thus there is a need for innovative organisation of resources.  This study will examine a model where midwives from the governmental hospitals are deployed to reach out weekly to remote villages, to give ante- and post-natal care. The study aims to monitor if this model will raise the women?s use of the health services by improving access to qualified care providers; midwives. 

The research topics focus on sustainable improvements of maternal health and health systems. The study will be carried out in collaboration with Norwegian research institutions. The investigation aims to provide new knowledge about organising continuity of care in a low-middle income, military occupied country. The study may contribute to universal knowledge also important for high-income countries striving to fill the gap between primary and secondary health systems. Attempts to overcome these challenges are made in many high-income countries, like the Norwegian "Interaction Reform" from 2012, to enhance the collaboration between primary and secondary health care. 
The fragmentation of health services constitutes a modern worldwide challenge that can threaten health. This study may provide new knowledge that contributes to overcome this challenge.

Validation of a Continuity of Midwifery Care Model in Palestine

Den palestinske perineum og fødselskomplikasjon studien er et samarbeid mellom Oslo universitetssykehus (OUS), the Palestinian Ministry of Health, Birzeit universitet og Universitetet i Oslo (UiO), og er forankret ved Intervensjonssenteret ved OUS Rikshospitalet. 

Tre palestinske gynekologer, dr Kaled Zimmo og dr Mohammed Zimmo fra Gaza samt dr Hadeel Ali fra Ramallah på Vestbredden, har i perioden 1.mars 2015 til 30.april 2017 samlet inn den største databasen på mor-barn helse i Palestina noen gang. Studien inkluderer tre statlige sykehus i Gaza og tre på Vestbredden, og inneholder informasjon om 75 000 gravide og fødende palestinske kvinner. 

Dr Kaled Zimmo har i sin studie kartlagt episiotomi praksis i Palestina. Episiotomi innebærer å skjære muskler i perineum for å gi bedre plassforhold når barnet fødes. Dette er den vanligste kirurgiske prosedyren ved fødsel, men kan føre til senskader og skal derfor ikke gjøres rutinemessig ved alle fødsler. Dette doktorgradsarbeidet studerte kunnskapsnivået til palestinske fødselshjelpere før og etter ekspert-opplæring i diagnostikk og behandling av fødselsrifter, og kartla gjeldende episiotomi praksis i Palestina.  I tillegg undersøkte man effekten av to ulike intervensjoner utviklet for å forebygge episiotomi ved å bruke en to-hånd-støtte-teknikk av perineum. 
Dette doktorgradsarbeidet viste at episiotomi blir utført rutinemessig på kvinner som føder vaginalt for første gang i Palestina. Videre demonstrertes klart den positive effekten av opplæring for å bedre utførelsen av episiotomi, av diagnostikk og behandling av perineal-skader og av intervensjoner ment å redusere forekomsten av episiotomi. 

Dr Mohamed Zimmo studerte bruken av keisersnitt i Palestina. Keisersnitt er en av verdens vanligste kirurgiske prosedyrer. Operasjonen kan få uheldige konsekvenser for både mor og barn på kort og lang sikt. Forekomsten av keisersnitt varierer fra land til land, men også fra sykehus til sykehus i samme land. For å kunne forstå mulige årsaker til den alarmerende økningen i forekomsten av keisersnitt globalt, er det viktig å identifisere hvilke kvinner som har høyest risiko for keisersnitt. Resultater viste store variasjoner mellom sykehusene i forekomster av keisersnitt. Tidligere keisersnitt eller prøverørsbehandling var assosiert med høyre forekomst av keisersnitt, men kunne ikke forklare forskjellene i forekomst mellom sykehusene. Omtrent 60 prosent av variasjonen i forekomst kunne forklares med ulikheter i oppgitte årsaker til keisersnitt. Mer enn 40 prosent av keisersnittene ble utført på kvinner som tidligere har hatt keisersnitt, 12 prosent hadde tvillingsvangerskap, og 8 prosent fødte for tidlig (<37 svangerskapsuker). Tiltak kan nå settes inn mot å øke andelen normale fødsler etter keisersnitt og ved å redusere antall keisersnitt ved tvillingfødsler og tidligfødsler.

Dr Hadeel Ali studerte alvorlige fødselsrifter i Palestina. Alvorlige fødselsrifter (obstetric anal sphincter injuries, OASIS) er en komplikasjon ved vaginal fødsel og medfører skade av endetarmens lukkemuskel med eller uten skade på selve endetarmen. OASIS kan medføre underlivssmerter, gi problemer med å ha sex eller kan føre til at kvinnene mister kontroll over luftavgang og avføring.  Avdekking av fødselsrifter umiddelbart etter fødsel vil redusere negative konsekvenser, men forebygging er den optimale strategien. 

Fødselsrifter har blitt neglisjert i Palestina med begrunnelsen at det forekommer sjelden. I sin avhandling har Hadeel Ali og medarbeidere undersøkt effekter av forskjellige opplærings-metoder for bedring av diagnostisering og forebygging av fødselsrifter ved seks statlige fødselsklinikker i Palestina. Kvaliteten på episiotomi, klipping ment å utvide vaginalåpningen ved fødsel, ble også undersøkt.

Ali og medarbeidere fant at oppdagelsen av fødselsrifter økte etter at jordmødre og leger fikk teoretisk og praktisk opplæring i korrekt diagnostikk av behandling. I tillegg fant man at kvaliteten på episiotomi var dårlig og at den bidro til økt risiko for fødselsrifter. Etter opplæring både i korrekt utførelse av klipping og i bruk av to-hånd-støttegrep for å beskytte perineum gjennom animasjon i tillegg til individuell opplæring, fant man for det første at animasjon alene reduserte forekomsten med 50 prosent. Når man i tillegg gjennomførte individuell opplæring i to-hånd-teknikken, ble forekomsten av OASIS ytterligere redusert med 50 prosent. Hovedfunnet er at forekomsten av fødselsrifter kan forebygges effektivt gjennom bruk av ulike opplæringsmetoder, noe som gir bedret fødselshjelp og reduksjon av unødvendige helseplager. 

Metodene beskrevet her anvendes nå på palestinske sykehus på Vestbredden og i Gaza.

The main aim is to reduce the incidence of perineal tears during vaginal delivery in Palestine, introducing a new support technique of perineum. Norwegian intervention studies have shown that the incidence of obstetric anal sphincter injuries (OASIS) can  be reduced by 40-70% by improving hands-on delivering techniques. The reduction was achieved by educating all staff on the delivery unit, implementing a new hands-on delivering technique. Some previous research has shown that non-interactive training by a nimated instructions or videos? in emergency obstetric care, might be equally effective as conventional interactive hands-on training. However,the effectiveness of mHealth training compared to conventional training programs remains to be assessed properly . By performing a multicentre intervention study, we will explore the effect of interactive hands-on training? on the incidence of perineal tears. Likewise we will study effect of non-interactive training by animated instructions. Three hospitals on the W est Bank (WB) and three in Gaza have accepted to participate. We will also investigate whether health personnel?s attitudes towards training methods influence the results. Intervention1 (6 months): Education in diagnosis and repair and implementation of s ystem to register data. Observation1 (6 months): Data-registration baseline. Intervention2 (6 months): One hospital in Gaza and one in the WB will receive ?interactive bedside training in hands-on manual perineal protection? for two weeks, whereas two oth er hospitals will have ?non-interactive training in hands-on manual perineal protection? by animated instructions on smartphones. The control units will be another two hospitals without training during the study period. Observation 2 (12 months): There wi ll be a 12 month observational period where all data is collected. Post-study intervention (6 months): Control hospitals will be offered bed-side interactive training in the same technique.

Reimagining the Operating Room - DP091

KINT er et samarbeidsprosjekt mellom Intervensjonssenteret (OUS), institutt for informatikk (UiO) og institutt for innovasjon og økonomisk organisering (BI). Prosjektet skal bidra til VERDIKTs mål om innovasjon gjennom å styrke implementeringen av Interve nsjonssenterets (OUS) TAVI-prosjekt, en ny behandlingsform for alvorlige hjertelidelser. Metodikken som utvikles i prosjektet vil i neste omgang kunne anvendes for å styrke implementeringen av andre tilsvarende høyteknologiske medisinske prosedyrer i nors ke sykehus. 

Prosjektet omhandler tingenes internett gjennom å se på hvordan teknologier og digitale informasjonskilder må sammenkobles for å lykkes med TAVI-prosjektet. Dette forutsetter at prosedyren gjennomføres av tverrfaglige team, hvilket medfører  betydelige endringer i organisering og tverrfaglig praksis. Det er av stor betydning for TAVIs suksess å kunne støtte samspillet mellom nye teknologikombinasjoner og den praksisen de skal inngå i. Prosjektet vil ligger derfor innenfor fagsøyle fire, med f okus på utfordringer knyttet til utvikling og implementering av ny teknologi, grunnet dagens organisering, regulering og institusjonelle rammer i helsevesenet. 

For å sikre høy kvalitet på det arbeidet som skal gjøres i prosjektet vil det være samarbeids relasjoner til flere ledende internasjonale forskningsmiljøer. Det vil legges stor vekt på formidling av resultater til brukere i helsevesenet, forskningsmiljøer gjennom publikasjoner i ledende internasjonale tidsskrift, studenter og offentligheten.

Fra lokale gjennombrudd i kunnskap til integrasjon i medisinsk praksis (KINT)

Republican Tuberculosis (TB) Dispensory, Petrozavodsk er ledet av Dr. Anna Belozarova, som har tidligere har hatt kontakt med Det Norske Radiumhospital og Russlandsenteret gjennom et langvarig samarbeid i radiologi. Lederne for Intervensjonssenteret og Ru sslandsenteret besøkte henne på TB Dispensory i sommer med tanke på samarbeid innen nye medisinske områder. De ble da presentert for en lovende student som vi nå søker om å få til Norge.

Petrozavosk i Russland er et fattig gammelt industristed med mye fo rurensning og høy forekomst av sykdommer som kreft, HIV, tuberkulose og andre luftveisinfeksjoner. Leder for lungeavdelingen ved Rikshospitalet (RH) er en kapasistet i Norge på lungesykdom og miljøproblematikk. Intervensjonssenteret, i samarbeid med lunge avdelingen ved Rikshospitalet (RH) har i 10 år arbeidet med behandling via luftveiene, dvs. terapeutisk bronkoskopi. Senteret gjør ca. 100 slike inngrep i året og i omtrent 90 % av tilfellene er det kreft. Intervensjonssenteret begynte i sommer med ultral ydundersøkelser av lymfeknuter utenfor luftveiene. Det er behov for å utvikle en teknikk for å ta biopsier av disse lymfe-knutene ved sykdom nær luftveiene, da det er et vanskelig å avgjøre om luftveiene er infiltrert av tumor eller om de bare er komprime rte. CT har lav sensitivitet og spesifisitet når det gjelder diagnose av lidelser i mediastinum og stadiebestemmelse av lungekreft, og mediastinoskopi er en invasiv metode som bare når et fåtall av de lymfeknutene man ønsker å nå. En doktorgrads-student k an være med på å utvikle denne teknikken

Et annet prosjekt som det er aktuelt å involvere studenten i er 
utredning og etablering av en hjerte-lungemaskin til bruk under operasjoner. Det er vanlig å bruke en oksygenator for å erstatte lungefunksjonen før  transplantasjon, men det er ikke brukt tidligere til intervensjonsbronkoskopi av pasienter med kritiske luftveisstenoser. Metoden vil kunne bidra til at det gis et tryggere tilbud til nye pasientgrupper.

Funksjonell MR ved Intervensjonssenteret

Studien utgår fra Program for utvikling av hjertekirurgi ved Thoraxkirurgisk avdeling i samarbeid med Intervensjonssenteret ved Rikshospitalet, og sammenligner pasienterfart resultat etter kransarteriekirurgi hvor inngrepet er gjort enten med (konvensjone lt) eller uten (ny metode) bruk av hjerte-lungemaskin under operasjonen. Dersom kirurgi uten bruk av hjerte-lungemaskin gir mindre gass- og partikkelbelastning til hjernen, men samme bedring av blodforsyningen til hjertets kransarterier, er det mulig at d ette også gir bedre pasienterfart resultat målt som selvrapportert helserelatert livskvalitet. En randomisert studie ble startet i 1999 for å få fram en systematisk og vitenskapelig metodevurdering, både med objektive resultatmål og pasientens egen evalue ring. 
I alt 120 pasienter inngår i studien og er fulgt opp med spørreskjema 3, 6 og 12 måneder etter operasjon. I tillegg til spørreskjemaundersøkelsen gjøres en fenomenologisk delstudie der ca. 20 pasienter er dybdeintervjuet ett år etter operasjon. Stu dien har internasjonal betydning ved å være en stor, tverrfaglig og single-center randomisert studie av den nye operasjonsmetoden. Datasamling er gjennomført, og det er ønskelig å frikjøpe tid i den arbeidsintensive analysefasen for å avslutte prosjektet.
