Personalized medicine in general, and modern gene sequencing technologies in particular (High throughput sequencing, - HTS), is advancing at rapid speed, which implies that implementation of these technologies is to be "aiming at a moving target". Development of competencies to deploy technologies and knowledge developed externally is therefore as important as internally developed solutions. However, own development is probably a good path to develop these competencies. This project, which we have named "geniOUS", aim to accomplish both. Development of solutions deploying HTS technologies in practical /clinical use requires that the solutions are integrated in existing infrastructure, both technologies and work processes. This entails both IT infrastrucure as well as legal frameworks. In this project ("geniOUS") the following has been accomplished until now: -The previously developed decision support system "Ella" has been further developed. "Ella" was first deployed in production at "unit for cancer genetics" (UCG), which has resulted in considerable increased efficiency in the interpretation processes. Then Unit for general genetics (UGG) followed, which required more flexible solutions than at ECG. Following full re-factorization of the source code, this is now far more flexible with respect to filtering, "dynamic gene panels" and simplified re-analysis. St. Olavs hospital is now about to deploy "Ella", and interest from "Danish sequencing Centre has been received. - At USIT, several technological developments have been conducted to increase functionality, capacity and flexibility in analyses of gene variation. USIT has now finalized a solution to manage "dynamic consent". However, for regualtory reasons, an additional project managed by section for QC at AMG has to be completed before this solution can be deployed. -There has been conducted a substantial "mapping effort" of the legal landscape, as it is evident that the current legal framework is inadequate to manage the challenges we are facing. Over and above Norwegian legislation, international treaties are demonstrating increasing impact on our legal position, e.g. GDPR. A PhD dissertation has been completed, which is a major contribution to this legal field.. As these legal challenges are not limited to Norwegian conditions, a Nordic network has been established within this legal field of personalized medicine. The legal section at OUS is also strongly involved in this network. -Within health economics, focus has first been aimed at solutions for evaluation of solutions for analysis of heritable dispositions for breast- and ovary cancer. However, simpler models for clearly defined issues can be expanded to more complex situations with more experience. The first scientific article has now been published on the topic, and two additional papers are in process. The candidate has completed a study visit at University of Oxford for three months. -An important area where we want to deploy HTS is within pharmacogenomics. "geniOUS" is now working closely with Department of Pharmacology at OUS on such solutions. Additional innovation funding from Regional Health Authorities has been granted for this. A prototype has been developed, and the intention is now to integrate the solution in DIPS Arena. -"geniOUS" is working closely with the "BIGMED project". The latter project has a wider scope than "geniOUS", but there are obvious interfaces, especially within the area of active patient contact and "dynamic consent". As of today it appears the solution from USIT is the best candidate to pilot such solutions. Final administrative clearance of who will manage a communication solution towards external users (patients) is still a challenge outside the control of the project. Over and above this, there are now initiatives running to further expand the international network, i.a. with Global alliance for genomics & health (GA4GH; ga4gh.org).

1) Verktøyet for varianter "Ella" er videreutviklet med betydelige nye funksjonaliteter og er implementert ved flere enheter ved AMG, og har medført en kvalitets- og effektivitetsheving. Det er også i ferd med å implementeres på andre HF, og har vakt internasjonal interesse. 2) USIT har utviklet en løsning for "dynamisk samtykke", og kommunikasjonsskjemaer er utviklet og forankret i klinisk og juridisk. 3) Det er ferdigstilt et doktorgradsarbeid innen helseøkonomi fokusert på verdien av HTs baserte analyser. Ut over dette er nå AMG ved OUS akkreditert for å kunne yte HTS baserte kliniske analyser. 4) Det ferdigstilt en doktorgrad som er sentralt grunnlag for utviklingen av det legale rammeverket i Norge. Det er også etablert et nordiske nettverk. Samlet sett mener vi prosjektet har vært instrumentelt i å utvikle både gode teknologier og sterke tverrfaglige miljøer som er grunnlaget for innføring og videreutvikling av presisjonsmedisin i Norge

Implementering av moderne gensekvenseringsteknologi ("High throughput sequencing" - HTS) i klinisk medisin for gradvis å realisere en visjon om "persontilpasset medisin" krever utvikling av tilpasset IT beslutningsstøtteverktøy, oppbygging av brede tverrfaglige kompetansemiljøer, samt nye mekanismer for interaktiv bruk av pasientdata (personsensitiv informasjon) for dynamisk å videreutvikle vår forståelse av betydningen av ulik genetisk variasjon. Denne siste fordringen krever helt nye mekanismer for hvordan vi involverer brukerne/pasientene dynamisk i kvalitetssikring, metodeutvikling og translasjonsforskning. Dette reiser ut over rent IT-tekniske utfordringer også en serie rettslige spørsmål og utfordringer som må vurderes og løses for adekvat implementeringen av denne teknologien. Teknologien gir helt nye muligheter for brukermedvirkning og aktiv involvering av samfunnet i utviklingen av helsevesenet, noe som også får betydning for de juridiske spørsmålene. En sentral utfordring er å omforme de enorme datamengdene fra DNA-sekvensering til til beslutningstøttende kunnskap. Som et munnhell sies at vi har "the 1000 dollar genome, and the million dollar interpretation". Vi er i tiltakende grad avhengige av å bruke lokalt akkumulerte resultater fra andre personers prøver som sammenlikningsgrunnlag for å tolke genetiske svar. Forskningsutfordringen i prosjektet vil i høy grad være tverrfaglig, preget av translasjonsforskning. Problemstillingene kan struktureres langs fire hovedsøyler: -Videreutvikling og gradvis automatisering av beslutningsstøtte for tolkning av gendata for ulike kliniske tilstander -Utvikle kommunikasjonsløsning for pasienters informerte medbestemmelse -Mer strukturerte metoder for medisinsk metodeutvikling og evaluering, herunder helseøkonomiske metoder. -Juridiske spørsmålstillinger relatert til analyse og kommunikasjons direkte med pasientene, og for bruk av pasientenes data som sammenlikningsgrunnlag for analyse av andre pasienters genomer.
