Person-centered co-production Living Lab ecosystems: development, evaluation, sustainable Point-of-Care services to empower self-management

Our vision is to implement a set of state of the art ICT-based point-of-care services for home-living users, meeting the needs of future of integrated care through digital collaboration. This vision entails co-creation and co-design of a set of sustainable generic digital health services for users with different complex medical conditions. The project ideas build up on experience developed through European projects, national and regional projects, in Agder, where these projects have been carried out in close collaboration with other national and international partners. During the pre-project period, activities to contact relevant national partners with completive ideas and competence, and existing international networks will be used to obtain multi-disciplinary competence in the consortium. We have established a relevant project management group of 5 people being responsible for the pre-project application. They are leading five working groups with 20 experts representing hospital and municipality health care sector, health-ICT partners, industrial partners, research partners and consulting representatives from end-users. The working groups will be expanded in 12 Work Packages, defining the research questions, methods, actual tasks and milestones, and a plan for deliverables. Actual partners with relevant competence will contribute in defining the WP. Methods for incorporating a watchdog process in the Lighthouse project will make sure that new innovative ideas will be incorporated and developed, as well as new state-of-the-art technology solutions, and according to new treatment procedures and paradigms. Important actions will be innovation and evolvements in the health and care service chain, where a focus will be on efficiency and effectual collaborative support with easy access to relevant information, and utilization of sufficient ICT tools to support actual workflow and individual treatment plans, supported by quality indicator measures.
