Prediabetes and type 2 DIAbetes in South Asian immigrants. Disease Perception, Pathophysiology, Prevention and Treatment - The DIASA program

The DIASA Study design: A 6-step combination of single and multi-center cross-sectional and longitudinal observational studies and prospective treatment intervention trials, using quantitative and qualitative research methods. Step 1: Multi-centre, cross-sectional investigation of glucose metabolism by oral glucose tolerance tests to find the prevalence of prediabetes and type 2 diabetes (T2D) in women of Norwegian and South Asian ethnicity with previous gestational diabetes. Qualitative examination of disease perception, possibilities for and barriers against T2D prevention. Step 2: Single-centre cross-sectional investigation of hepatic glucose Production (HGP), hepatic and peripheral insulin sensitivity and insulin secretion in Norwegian and South Asian women with prediabetes (impaired glucose tolerance,IFG or impaired fasting glucose,IGT). Step 3: Single-Centre 12 week randomized, blinded pilot intervention trial of the effect of 4 different oral antidiabetic agents (OAD) on HGP in women with prediabetes (IGT or IFG). Step 4: Establishment of a South Asian T2D cohort in General Practice with biannual registration of treatment, blood glucose control, blood pressure, lipids and complications. Step 5: Investigation of treatment compliance in the South Asian T2D cohort using both quantitative and qualitative research metods and linking to the Norwegian Prescription Registry. Step 6: 1-year randomized double-blind, placebo-controlled intervention trial with the two most effective OAD agents from step 3 versus standard of care in 120 T2D men and women of South Asian ethnicity, also applying new knowledge about disease perception and treatment compliance from the earlier DIASA projects. The most critical challenge associated with establishing the DIASA program is the recruitment of collaborators to the DIASA 4, 5 and 6 projects in General Practice. The Project Establishment Support Application is therefore intended mainly for funding of this recruitment process.
