Digital trening som medisin

Increasing age in the population is a big challenge for the health authorities. Regular physical activity has great potential for the prevention of disease in the aging population and the most prominent diseases. The use of regular physical activity is an effective treatment for several age-related diseases. The parent idea has been to develop and evaluate the digital tool Myworkout GO as a new product including services for tomorrow's exercise as medicine and prevention of chronic diseases that might help improve capacity within today´s healthcare system. The project has been a cooperation between business (Myworkout AS, Storebrand Helse AS, Klinikk for Alle AS) and research (SINTEF, Myworkout AS) and the use of scientific methods within technology, medicine, physiology, service design and user-controlled innovation. Home-based exercise with a minimum requirement for equipment has been developed to make it possible to quantify cardiovascular capacity and muscular leg strength and to carry out an effective exercise that develops leg strength with no other assistance than an app. In the project, a prototype for equipment-independent effective strength and conditioning training has been developed. The project has succeeded in making an equipment-independent self-service estimation of 1RM based on one-leg squats, and calculation of biological age based on this. The project has also succeeded in developing a prototype of step training for the evaluation of maximum oxygen uptake and biological age using a smartphone that registers sensor data from the phone. This technology is verified with less than a 10% margin of error, and can thus be used for medical purposes. The project's biggest technological breakthrough is HIT Live. This technology is verified for use for both commercial and medical purposes and measures the user's training intensity in real-time, and guides and motivates the user to achieve high-intensity training minutes (HIT minutes) equivalent to 4x4, but independent of the type of training protocol. Natural HIT intervals will be a central part of the user journey in Myworkout, which focuses on guiding the user to achieve high-intensity training minutes, which is documented to be directly related to health. The HIT minutes work on all smartphones without the need for a heart rate sensor or equipment other than a smartphone. This means that everyone, with this technology, can have their activity measured as a health outcome, which has not been possible before. The Norwegian Directorate of Health has contacted Myworkout with a request to become the first pilot for approval of Medical Device in Norway. A large validation study of the app against direct measurement of maximum oxygen uptake (VO2max) has been carried out (n = 160). To gather user experiences and try out the commercial model, a test implementation was launched in Myworkout GO, which showed that the concept was viable in that users chose to pay and use the solution. Norway's first agreement on the purchase of licenses for digital training as a medicine with a public health authority has also been closed, where the licenses will be made available to patients and healthcare professionals via Helsenorge.no. Furthermore, standardized test protocols have been established for evaluation, and a study has been carried out (n = 20) with a comparison of the effect of training on a treadmill with an instructor and with an app outside / at home on your own. The development of the service model is built on qualitative interviews with participants, health personnel, NAV, and insurance companies. The participating actors' knowledge of and experiences with Myworkout GO and the connected rehabilitation service are described. Experiences with the current service and technology have been used as a background for describing a service model related to the technology, and which includes the involvement of the various actors before, during, and after the rehabilitation stay.

Såvidt vi kjenner til er dette den første teknologien som har gjort. - Inngått første kommersielle avtale med offentlig helseforetak, der pasienter og helsepersonell får lisenstilgang betalt av staten. - Inngått første kommersielle avtale med forsikringsselskap i Norge og Sverige. - Oppnådd offentlig godkjenning som digital trening som medisin på helsenorge.no med høyeste kvalitetsscore "A".

Befolkningsvekst, økt levealder og økt forekomst av livsstilssykdommer er faktorer som vil gi helsevesenet store utfordringer i fremtiden, hvis man ikke endrer måten man gir behandling på. Det er behov for både å forebygge forekomsten av denne typen sykdommer, gi bedre behandling til de som har blitt syke og effektivisere måten man tilbyr behandling på. Dette er utfordringer som prosjektet tar tak i. I prosjektet tar man for seg behandling av kronisk syke (hjerte-, lunge- og revmatisk sykdom) som case, men løsningen vil kunne brukes både i behandling av andre sykdommer og av privatpersoner som forebygging. Forskningsbehovet som er knyttet til utvikling av ny teknologi i MyworkoutGO og utvikling av en tjenestemodell knyttet til teknologien, som på samme tid er tilpasset brukernes behov, er bredt sammensatt. Følgende problemområder krever forskning for å realisere innovasjonen, og sentrale nyhetselementer inkluderer: A) Ny kunnskap om hvordan teknologien (MyworkoutGO) skal utvikles for å løse behovet til både bruker og helsepersonell. B) Ny kunnskap om brukerbehov og hvordan tjenestetilbudet må fungere for å gi ønsket effekt. C) Ny kunnskap om systematisk evaluering og dokumentasjon av forbedring. Innovasjonen innebærer utvikling av nytt produkt med en tilhørende tjeneste for Myworkout, som skal bidra til å styrke merkevare og konkurranseevne både nasjonalt og internasjonalt. Prosjektet er et samarbeid mellom industriaktør (Myworkout AS, Klinikk for alle AS, Storebrand Helseforsikring AS), FoU (SINTEF, Myworkout AS).
