DELTAKELSE OG KVALITET - UTVIKLING AV METODE FOR Å STYRKE KVALITETEN I KOMMUNALE TJENESTER FOR BARN OG UNGE MED FUNKSJONSNEDSETTELSER.

Alternative title: PARTICIPATION AND QUALITY - DEVELOPING A METHOD FOR STRENGTENING THE QUALITY OF MUNICIPAL SERVICES FOR CHILDREN AND YOUTH WITH DISABILITIES

The project is a collaboration between the Norwegian National Advisory Unit on Disabilities in Children & Youth (NKBUF), SINTEF Digital, and the municipalities of Vestvågøy, Stjørdal, Askøy and Horten. The broad aim of the project was to contribute to improving the municipalities' provision for children and young people with disabilities. An important aim of the services for this target group is that children and young people, who are otherwise at risk of being neglected and passive, get the opportunity to participate in various areas such as school and leisure, and being included in social contexts. Children and young people with disabilities have the right to services that will contribute to participation and inclusion, and the municipalities have a duty to facilitate and implement such services. Children and young people with disabilities, their parents and the service system all represent important expertise that have contributed to the implementation of the project and to find the best solutions for improving the quality of the services. An extended project group that included the project management and representative professional from all four participating municipalities was established early in the project. Anchoring of the project with the management of the municipalities was secured through a formal agreement. During the project period, a number of meetings and workshops (physical and digital) were organised, where the project management met the municipalities individually and together. The project has aimed to develop a method to improve the quality of services for children, young people and young adults with disabilities. Focus group discussions were carried out with children/young people, parents and service providers in the four municipalities. Results from the qualitative data show that children/youth/parents want an increased emphasis and better services, among others, in following areas: • Municipal service coordination • User participation • Digital information and communication services • Recruitment, training/guidance and follow-up of support contacts/BPA • Culture and activity offers and transport services • The school as an integrative and inclusive learning arena SWOT analyzes have been carried out in all municipalities, where a wide range of service providers with responsibility for the target group in the municipalities carried out a three-hour dialogue process based on questions from the project management. Finally, we carried out a survey among children/youth registered by the municipalities as being in need of services, in combination with a comparative random sample of children/youth in the same municipalities. The survey results showed that children with disabilities participate less than children without disabilities: They experience more barriers for participation, assess their own participation as less, are less content with own participation and report lower actual participation. Repeated surveys in the future will provide information on any development of level of participation and whether the gap between children and youth with and without disabilities have been affected. Feedback on the results from both the qualitative and quantitative data have been given to the project management in the participating municipalities. During the spring semester 2023, a workshop was held over two days with the participating municipalities, the management of NKBUF and representatives of the Norwegian Directorate of Health. This workshop forms the basis for how the experiences from the research can be implemented and put to use in municipalities in general. The results have been shared with relevant and responsible personnel in the municipalities who, together with the central project management and the users, entered into a dialogues on if and how the services should be changed in order to increase participation and inclusion in the target group. Over time, this can contribute to the development of new strategies that contribute to service innovation in the municipalities. Participation and inclusion should also be measured again over time to monitor changes. The experiences with this way of measuring should then be shared with other municipalities in Norway.

Som vi vektla i søknaden valgte vi å legge prosjektet til rette for at vi kunne oppnå en Radikal innovasjonsgevinst som vi knyttet til ønsket om å finne nye løsninger på komplekse utfordringer i kjølvannet av samfunnsutviklingen knyttet til målgruppen barn, unge og unge voksne med ulike variasjoner av funksjonsnedsettelser. Vårt prosjekt er styrt med tanke på å kunne oppnå en Radikal innovasjon, fordi vi har hatt som klart mål å endre praksis i kommunene, utvikle, validere og implementere nye verktøy som skal kunne si noe konkret om kvaliteten på de kommunale tjenestene til barn og unge med funksjonsnedsettelser og hvilke tjenester som må vektlegges for å kunne øke tjenestekvalitet. Vi har fått bekreftet gjennom sluttseminarene at de som har vært involvert i denne forskningsprosessen i deltakerkommunene har fått en høyere grad av bevissthet rundt kvalitetsarbeid i kommunen. De har fått ny kunnskap om hva brukerne strir med i hverdagslivet – og de har vært med på ulike prosesser som skal bidra til endring av praksis i tjenestetilbudet for målgruppen. Den typen kvalitetsmåling av disse tjenestene vi valgte å gjennomføre, er i liten grad utviklet, og tatt i bruk i praksis i offentlig sektor og vi har hatt som klar ambisjon å gi kommunene et redskap til å monitorere egne tjenester, både gjennom bruk av nye indikatorer som er blitt utviklet og testet i dette prosjektet, og eksisterende registerdata (IPLOS og KPR/kommunalt pasientregister). Kritiske suksessfaktorer er hvorvidt kommunene prioriterer å implementere den endrede praksis som prosjektprosessen har skapt grunnlag for – og i hvilken grad de nødvendige endringsprosessene gis de koordinerende rammebetingelsene de trenger for å bli en del av kommunens strategiske praksis.

Nasjonal Kompetansetjeneste for barn og unge med funksjonsnedsettelser har i sitt mandat forpliktet seg til å arbeide for å etablere et kvalitetsmålingssystem. I forarbeidet til prosjektet engasjerte vi mange organisasjoner; kommuner, brukerorganisasjoner og ulike kompetansemiljø. Disse var involvert i arbeidet med å utvikle struktur og innhold i et fremtidig nasjonalt kvalitetsmålingssystem for nevnte målgruppe. Spesialisthelsetjenesten har en rekke kvalitetsregistre. De aller fleste av disse er knyttet til en diagnosegruppe. Vi er kommet frem til at det ikke vil være mulig å bygge et kvalitetsregister i tradisjonell form, fordi vi i vår målgruppe har brukere som er hjemmehørende i de fleste av de diagnosegruppene det er bygd kvalitetsregistre rundt. Vi trenger, i tillegg til å innhente data fra de eksiterende diagnoserelaterte registrene, et kvalitetsmålingssytem som kan innhente kvalitative data om barn og unge med funksjonsnedsettelser sin funksjon i hverdagen, med spesiell vekt på data som beskriver deres sosiale deltakelse sammen med andre i de ulike sammfunnssettingene de inngår i. Vi ser på deltakelse som en menneskelig handling som foregår under konkrete betingelser sammen med andre i sosial praksis i forhold til en felles sak (Kissow & Klasson 2018) Vi vil kartlegge betingelsene som de sosiale og fysiske omgivelsene gir vår målgruppe i en kombinasjon med de forutsetningene som individet har. Skal vi kunne utvikle, teste og evaluere et kvalitetsmålingssystem bygd på målgruppens hjemmeliv, menneskelige og personlige relasjoner, sentrale livsområder (utdanning, arbeid og økonomi) samt samfunnsdeltakelse og sosialt liv (Witsø 2014), har vi valgt å gjøre det sammen med brukerne og kommunene. I 2018 etablerte vi Mønsterkommunenettverk med fire kommuner. I et samarbeid mellom disse, Kompetansetjenesten og SINTEF, vil vi gjennomføre et fireårig prosjekt som skal gi oss et kvalitetsmålingssystem som kan tas i bruk av resten av landets kommuner.
