Sammen om god oral helse hos barn og ungdom utsatt for vold og overgrep – et prosjekt fra Statens Barnehus.

Violence and abuse can have serious health consequences. The effect of violence and abuse on the oral health of the vulnerable children is an area with big knowledge gaps. However, there are evidence that children with violence and abuse experience have poorer oral health than the general population and there is also reason to believe that poor this has a negative effect on their overall health and quality of life. In Norway, the public dental health service is the only health service in the country that sees all children on a regular basis. Public dental health service has a lot of experience and knowledge about children's dental health and its role is therefore central to both the detection and prevention of poor oral health in the vulnerable children. In this project, we focus on children who, at the request of prosecuting authorities, have been examined at the State Children's House by dentists in cases where there was suspicion of violence and / or sexual abuse. Our experience shows that dentists find it difficult to follow up the children and their need for dental care after the children have been examined at the State Children's House. As of today, there are no routines for how the information is disseminated to the public dental service. Therefore, it is not known whether the children receive the dental help they are entitled to. In this project, we want to investigate whether children's right to health care is safeguarded and how the children are followed up in the public dental service after being examined at the State Children's House. We also want to include the perspective of children and adolescents (user participation) on their own oral health. The knowledge we develop in this project will form the basis for the development of follow-up routines for vulnerable children after examination at the State Children's House and to ensure the best possible oral health in children and adolescents in vulnerable life situations.

Vold og overgrep i barndommen kan føre til alvorlige helsemessige konsekvensene. Effekten av vold og overgrep på den orale helsen til de utsatte barna et lite utforsket området, men de få studier på dette temaet viser likevel at barn med volds- og overgrepserfaringer har høyere forekomst av ubehandlet tannråte, dårligere munnhygiene, dårligere munnhelse og mer tapte avtaler innen helsehjelp enn befolkningen generelt. Det er derfor grunn til å tro at dårlig oral helse hos barn i sårbare livssituasjoner vil også ha en negativ effekt på deres generell helse og livskvalitet. Den nylig publiserte litteraturoppsummeringen på sammenheng mellom barnemishandling og oral helse påpeker videre at det finnes få kvalitative eller deltakende studier hvor barnas egne erfaringer og oppfatning av sin oral helse settes i fokus. I Norge er den offentlig tannhelsetjenesten den eneste helsetjenesten i landet som ser alle barn på regelmessig basis og tjenesten har derfor mye erfaring og kunnskap om barnas tannhelse. Fra 2016 har tannlegene blitt implementert ved de fleste Statens barnehus i Norge. Våre erfaringene med denne implementeringen viser at tannlegene har det vanskelig å følge opp barna og deres behov for tannbehandling etter at barna har vært undersøkt ved Statens barnehus. Det finnes per i dag ikke noen rutiner på hvordan informasjonen formidles til den offentlige tannhelsetjenesten. Derfor er det heller ikke kjent om barna får den tannhelsehjelpen de har rett på. I dette prosjektet ønsker vi å undersøke om barns rett til helsehjelpen er ivaretatt og hvordan barna følges opp i den offentlige tannhelsetjenesten etter å ha vært på Statens barnehus. Vi ønsker videre å inkludere barnas og ungdommens perspektiv (brukermedvirkning) på egen oral helse. Kunnskapen vi utvikler i dette prosjektet skal danne et grunnlag for utvikling av tiltak om behandlingsplan og måten man bør følger de utsatte barna etter undersøkelse ved Statens barnehus i den offentlige tannhelsetjenesten.
