Digital poliklinikk med hjemmeoppfølging

Smarter use of digital solutions makes it easier for patients to receive treatment at home, reducing the need for physical attendance at the hospital and ensuring that any changes in the patient's condition are detected earlier. This project tests different models of digital outpatient clinics by promoting the digitalisation of selected outpatient clinics at Oslo University Hospital (OUS) and the University Hospital of Northern Norway (UNN). In accordance with the National Health and Hospital Plan, the project aims to enhance the evidence base for hospital at home services in Norway. The Norwegian health technology company Dignio is responsible for the project. The project aims to define a new methodology for remote monitoring of outpatients at hospitals, as well as to further develop Dignio's platform so that it supports this methodology. With remote patient monitoring, the patient gets access to an app that connects to medical measuring equipment and a clinician’s tool for monitoring at the hospital. Via the app, the patient can report symptoms, send in medical measurements, monitor their own history and communicate with healthcare personnel via message or video. Healthcare personnel can monitor patients via a separate clinician platform and can give advice or intervene early in the event of a complication or signs of deterioration. This makes it easier to offer outpatients services that are needs-based rather than calender-based. The project involves several patient groups at two hospitals. At the Cancer Department at UNN, patients with cancer will be followed for a longer period of time. At OUS, several different patient groups will be followed, including patients from the Department of Neurology, the Pulmonary Department and the Department for Pain Management. Funded by The Norwegian Research Council's call for Covid-19, the project will examine whether the patients achieve a reduced risk of infection and higher levels of engagement and reassurance despite the pandemic situation, as well as improved self-management of their (often chronic) condition. The benefits for the health service are expected to be, among other things, improved prioritisation of scarce resources in a pressured situation. During the project period, Dignio has collaborated with the four departments from OUS and UNN as well as the Intervention Center to define patient groups and highlight challenges with the current course of action in traditional outpatient clinics. With inspiration from service design methodology, the project has described patient journeys for the relevant patient groups in the project, in order to identify needs and to be patient flows with remote patient monitoring. Furthermore, the project group has specified how the various patient groups are to be followed up at home with the help of the app: self-reporting of various measurements taken at home, questionnaires, use of the chat function, etc. The project group has defined content and procedures for how the clinics should follow up and handle self-reports from the patient. In parallel, the project has worked to adapt Dignio's platform so that the specific needs that come from using the solution in an outpatient context/hospital environment better can be supported by the platform. As part of the project, a post.doc was recruited in 2021 to lead the follow-up research of the project. A lot of time and effort was spent in planning the follow-up research, including obtaining the formal approvals to conduct the research and prepare the platform including getting approval for regulatory perspectives. At the start of 2022, the first patient was onboarded and the by the summer of 2023, all 104 patients were finished with their digital follow up. During this period, continuous adaptation of the platform was undertaken as well as follow-up research was performed. Dignio's platform is today, after the project has now ended, much better equipped to support a digital outpatient clinic at a hospital. The project has generated a lot of data, insight and experiences about digital home monitoring - these results of the project will be published in peer-reviewed journals.

Prosjektet har blitt gjennomført etter planen Prosjektet har hatt to hovedformål: Å tilpasse en løsning/metodikk for digital hjemmeoppfølging av polikliniske pasienter ved sykehus samt å gjennomføre følgeforskning. Per dags dato (sommer 2023) er både metodikk og den tekniske løsningen betydelig mer tilpasset behovet for å understøtte oppfølging av pasienter ved en digital poliklinikk på sykehus, enn hva den var da prosjektet startet i 2020. Gjennom prosjektperioden har vi også gjennomført forskning og samlet inn mye data, både kvalitative og kvantitative data. Disse forskningsresultatene vil bli publisert fortløpende i 2023/2024. Vi forventer fortsatt at prosjektet vil bidra med mye kunnskap om digital poliklinikk ved norske sykehus. Dette, i en fase da flere norske sykehus har gjennomført prosjekter på digital hjemmeoppfølging men ikke skalert/tatt i bruk digital hjemmeoppfølging som en del av ordinær drift. På nåværende tidspunkt er det for tidlig å konkludere med noen effekter, da dette vil bli publisert som en del av forskningen. Det kvalitative datagrunnlaget indikerer dog en meget positiv holdning blant pasientene, og flere avdelinger som deltok i prosjektet fortsetter med digital poliklinikk også etter at prosjektet er over.

Langt på vei stoppet poliklinisk oppfølging av pasienter opp ved landets sykehus i den første fasen av pandemibekjempelsen av korona-viruset. Viktig behandling for tusenvis av pasienter ble skjøvet på fordi det ikke var trygt å oppsøke sykehusene for de aller mest sårbare pasientene. «Over natten» gikk en rekke polikliniske avdelinger over til å benytte videokonsultasjon for å unngå fysisk nærkontakt. Prosjektet «Digital poliklinikk» skal ta dette til et nytt nivå, der videokonsultasjon kun er én del av en forbedret klinisk praksis for å øke trygghet og gi bedre behandling av polikliniske pasienter, et viktig skritt for å oppnå «pasientens helsevesen». Foruten gevinster for pasientene, skal prosjektet samskape en mer digital basert poliklinisk oppfølging for leger og sykepleiere i spesialisthelsetjenesten. Klinikere skal delta i å skape de nye arbeidsprosessene sammen med tjenestedesignere. Dignio skal videreutvikle sitt system for digital hjemmeoppfølging til å passe inn i de nye arbeidsmåten. Både klinisk utprøving og videreutvikling av systemet skal skje i samarbeid med Intervensjonssenteret på OUS som har et nasjonalt ansvar for å sikre trygg utvikling og innføring av nye metoder. Kreftpoliklinikken ved UNN vil delta sammen med poliklinikker ved OUS (lungeavdelingen, Avdeling for Smertebehandling samt Nevrologisk avdeling) for å teste ut løsningen i den daglige driften. En postdoktorstilling ved OUS som finansieres av prosjektet skal bistå med tjenestedesign og følgeforskning på hvilken effekt en slik poliklinisk behandling vil ha på pasienter, helsepersonell og organisasjon (inklusive bærekraftmål). Sykehuspartner og DIPS skal sikre et godt samspill mellom sykehusets eksisterende systemer og Dignios løsning for hjemmeoppfølging. Kreftforeningen deltar også som støttespiller og representant for pasientenes interesser.
