Automation of clinical genetic variant interpretation

DNA sequencing have undergone a technological revolution the past 15 years, and the costs of sequencing human genomes have fallen dramatically. In parallel, our understanding of how genetic variation contributes to disease development have been dramatically improved. Such knowledge can be exploited in genetic diagnostics and enable more personalized medical treatment. Therefore, there is considerable interest in implementing high throughput DNA sequencing into mainstream healthcare to the benefit of patients. The main challenge is to clinically interpret the vast amount of genetic variants that are identified by the method. The interpretation follows international guidelines and involves manual inspection of data. Genetic variant interpretation is the most important limiting factor for scalability. Today's diagnostic practices involves a "here and know" assessment of genomes. The lack of continuous monitoring of genomes represent an important limitation as several studies have demonstrated large increases in diagnostic yield if genomes are reanalysed regularly. This is caused by the rapid increase in our knowledge on how genetic variation influences disease susceptibility. Genetic variant classification is the most important bottleneck both for scalability and reanalysis. In this project we aim to gradually increase the level of automation of the variant classification process. More specifically we will focus on: 1. By analysing historical variant classification data, we will identify parts of the process that are amenable to automation and implement automation in our in-house developed decision support tool Ella. 2. Develop novel and improved tools for the exploitation of family segregation data in the variant classification process. 3. Increase the clinical utility of genome data by testing and developing solutions for automatic extraction of pharmacogenetic data of importance to our reaction to pharmacological treatments. We have in this reporting period recruited three people who will work in the project: Researcher Magnus Vigeland will lead the work on aim 2 and will also supervise the PhD candidate Christian Carrizosa who will work on aim 1 and 2. Silvio Armando Conandera has been recruited as a scientia fellow postdoc and will work on aim 3 of the proposal. Due to a protracted recruitment process we have sent a "change application" to NFR, requesting that the project period be prolonged with 6 months.

Dagens metode for kartlegging av DNA molekyler er >100.000 ganger mer effektive enn de vi hadde for femten år siden og innebærer at flere og flere pasienter får kartlagt alle sine gener (=hele sitt genom) som ledd i diagnostisk utredning av sykdom. Genomsekvensering generer flere millioner genetiske varianter som må tolkes for å finne ut om de kan forklare eksisterende sykdommer eller risiko for fremtidige sykdommer. Klinisk genetisk varianttolkning krever i dag mye manuell fortolkning og er den viktigste flaskehalsen for en bredere implementering av genomsekvensering i helsevesenet. I tillegg til å redusere skalérbarheten av genomisk diagnostikk er klinisk variantfortolkning også beheftet med kvalitets- og pasientsikkerhetsutfordringer. Flere studier har vist at en og samme genetiske variant relativt ofte tolkes ulikt i ulike laboratorier. For å øke skalérbarheten av genomsekvensering som diagnostisk verktøy og bedre kvalitet og pasientsikkerhet trenger vi å automatisere mer av varianttolkningsprosessen gjennom utvikling av IKT-beslutningsstøtteverktøy. I dette prosjektet ønsker vi å utvikle ulike slike beslutningsstøttemoduler som vil bli implementert i vårt in-house utviklete IKT-beslutningsstøtteverktøy Ella. Vi vil fokusere på: 1. Reanlayse av historiske variantklassifiseringsdata for å identifisere svakheter ved dagens algoritmer og hvilke deler av prosessen som egner seg for automatisering 2. Utvikling av ny metodologi for å inkludere familiehistorie som en integrert del av varianttolkningsprosessen 3. Utvikling av metodologi for automatisk ekstraksjon av farmakogenetisk informasjon som en integrert del av genomsik diagnostikk.
