One-size-fits-all is not true when it comes to drugs! In order to get a good effect vs side-effect balance the drug dose needs to be individually adapted. In this workshop, we will teach healthcare professionals to use modern computer programs to optimize patient's dose. The aim is to give health professionals basic knowledge about advanced dosing programs and what benefits and limitations they have. Participants can bring their own data on medicines they work with, and will get help from experts in the field to develop their own models. They will also learn how these models can be used to simulate the amount of drug in the body as well as testing different dosing strategies. Drug measurements from a large number of patients (population) are entered into the model and "what the body does with the drug" is determined for each patient; i.e. their ability to absorb and excrete the drug. New patients are compared with the population and values for the most similar, or a combination of several, previous patients is then used to calculate the optimal dose for the new patient. The computer program was developed by one of the teachers on the course and is for example used in the clinic to determine optimal doses of antibiotics in both children and adults and immunosuppressive drugs in organ transplant patients. The workshop runs over four days and the participants will live and work closely together. Since the participants can bring their own data to the course, this is a low-threshold offer to get started using dosing models that are relevant in their everyday work. Close professional contact between the participants and the teachers in combination with good socialization based on outdoor activities and joint dinners is intended to establish a lasting contact. Such a network will be of great help during further work with this type of advanced drug dosing. This philosophy has worked well in previous workshops that the teachers involved have completed elsewhere in the world.

Legemidlers farmakokinetikk beskriver "hva kroppen gjør med legemidlet", dvs. hvordan det blir absorbert, distribuert, brutt ned og eliminert fra kroppen. En nøyaktig beskrivelse av de farmakokinetiske prosessene til et legemiddel er nødvendig for å kunne dosere pasienter optimalt. Det finnes flere ulike måter å beskrive legemidlers farmakokinetikk, der den enkleste metoden baserer seg på gjennomsnittverdier for absorpsjon, distribusjon og eliminasjon. Det er imidlertid ingen pasienter som er gjennomsnittlige og det er derfor viktig å kunne beskrive hvordan en pasient skiller seg fra en annen så presist som mulig. Moderne datamaskiner gjøre det mulig ved hjelp av ikke-parametriske metoder å bestemme farmakokinetikken til hver enkelt pasient i en populasjon og kvantifisere hvordan pasientene i populasjonen forholder seg til hverandre. Ved å bruke slike modeller er det også mulig å inkludere individuelle kovariater som f.eks. nyrefunksjon, leverfunksjon, genetikk osv. for mer presis beskrivelse av farmakokinetikken til legemidlene. I workshoppen skal det fritt tilgjengelige, open source, programmet Pmetrics brukes. Direktøren ved laboratoriet, som har utviklet denne programvaren, vil være en av veilederne på workshopen og det vil bli lagt til rette for at deltakerne kan bruke egne data i de praktiske øvelsene. Deltakerne vil lære seg å modellere farmakokientiske data og hvordan modellene skal valideres. I vår gruppe har vi tidligere vist signifikant bedre treffprosent ved dosering av immundempnende legemidler til nyretransplanterte pasienter sammenlignet med erfarne klinikere. Denne type verktøy er hyppig brukt i klinikken for å individualisere doseringane av pasienter i land som Nederland, UK, Australia og USA. Deltakerne får også trenging i å simulere ved hjelp av populasjonsmodeller. F.eks. kan denne type modeller brukes til å simulere hvordan ulike doseringer av antibiotika klarer å oppnå bakteriedrepende eksponering.
