Ressursfordeling i helse- og omsorgstjenesten i kommunene

The primary health care system in the 356 municipalities in Norway is a key provider of safety and welfare, and has annual budgets of 150 billion NOK. We know how the municipalities are funded, and we can subsequently study how the resources are distributed to different services (such as GPs, physiotherapy, home care services, care homes, and nursing homes). However, we do not have enough knowledge about the internal resource allocation within the municipalities. Which models and criteria are used? What is the budgeting process like? The media shows cases of patients and relatives who do not get the services they are entitled to, and employees who must provide “stopwatch care”. But there are also examples of municipalities empowering patients by asking “What is important to you?”, and several trials of empowering employees through “trust models”. This research project will study the internal resource allocation within the health and care services in Norwegian municipalities. Further it will explore possible statistical relations between the models and processes for resource allocation, and cost effectiveness and quality of the services. The project will aim to develop a new management model for the health and care services in medium-sized municipalities in Norway. Here one must weigh the patients’ needs against the financial and professional framework of the service. Empowering of patients and employees must be seen up against the need for management, as well as weigh the demand for integral and coherent services against how the services must be provided from different organizational units. This model will be implemented in the municipality of Vestvågøy in Lofoten, which has a population of 11.500, and has professionally strong primary health and care services. The project will study what effects this management model can have on the quality of the service for the patients, on cost-effectiveness, and possible impact on employees and the organization as a whole.

Det synes å mangle kunnskap på intern ressursfordeling i helse- og omsorgstjenesten i kommunene. Tradisjonelt har kommunens helse- og omsorgstjeneste vært styrt gjennom rammebudsjettering. Inspirert av New Public Management (NPM) tok flere kommuner på 2000-tallet i bruk aktivitetsbasert finansiering (ABF). Her blir enhetene finansiert gjennom på forhånd spesifiserte kriterier. De siste årene har flere kommuner, inspirert av New Public Governance (NPG), gjort forsøk med samskaping, tjenestedesign og tillitsmodeller. Formålet med prosjektet er å kartlegge og analysere modeller og prosesser for ressursfordeling i kommunene. FoU-utfordring her er både å framskaffe kunnskap om modeller og prosesser i kommunene, og analysere mulige sammenhenger mellom dette og kvalitet og kostnadseffektivitet. En må her kontrollere mot kommunens størrelse, demografi, tjenesteprofil og inntektsnivå. Videre er formålet å bidra til ny styringsmodell for helse- og omsorgstjenesten i mellomstore kommuner. FoU-utfordringen vil både være å hente inn kunnskap og erfaring fra kommunene, samt å anvende kunnskap fra forsøk med samskaping, tjenestedesign og tillitsmodeller, både i kommuner og andre virksomheter (også internasjonalt). Det er videre en utfordring å kartlegge hvordan digitalisering, både av tjenesteproduksjon, i brukerdialog og med styringsdata i sanntid, kan bidra til dette. I utvikling av modellen må en avveie brukers behov opp mot gitte rammer, myndiggjøring opp mot behov for styring, og helhet og sammenheng opp mot tjenester fra ulike enheter. Endelig er formål å implementere ny styringsmodell for helse- og omsorgstjenesten i Vestvågøy kommune. FoU-utfordring vil både være knyttet til iverksetting, og til kartlegging av effektene på kvalitet, kostnadseffektivitet, og for organisasjonen. Dette vil ha preg av aksjonsforskning, der kandidaten både er deltaker og observatør, og dokumentasjon skjer innad mot deltakerne og organisasjonen, og utad i form av publisert forskning.
