Virtual health department at the municipality of Bodø The capacity of the Department of Health and Care Services (DHCS) in Bodø to follow-up patients transferred from the specialist healthcare to the primary healthcare sector is under increasing pressure. An aging population with complex and chronical medical conditions and introduction of the Coordination Reform, are two main contributing factors. The scope and complexity of the services needed in the in-home healthcare services are increasing. In addition, the different professions within the healthcare services are expected to deliver on-demand, quick and personalized healthcare. To meet these challenges, DHCS in Bodø established a Virtual Health Department (VHA) in 2021. The purpose is to prevent readmissions to hospitals and relieve the burden on short-term care institutions. In addition, improve competency levels in the in-home healthcare services and facilitate for a more streamlined patient transition between the different services. VHA is not a physical ward with inpatients. It is an alternative health service that allows for patient follow-up within their own homes, by a dedicated interdisciplinary team and the use of e-health and welfare technology. This includes remote monitoring as well as traditional home visits. Literature related to virtual health departments within the Norwegian context is scarce. Our project aims to systematically document and create new knowledge with regards to: - Which patient demography is particularly suitable for the VHA? - How can the nurse's clinical assessment be exercised in this setting? - How do the patients and their caretakers experience this service? The profession of nurses will be particularly studied to assess how their clinical perspective is challenged and made to adapt to new methodology in patient observation and evaluation. These experiences can in the long run contribute to training, recruiting and retaining of medical personnel in the primary healthcare.

Ifølge bestillerenheten i Bodø kommune har helse- og omsorgsavdelingen (HO) i dag manglende kapasitet til å følge opp pasienter som overflyttes fra spesialisthelsetjenesten til primærhelsetjenesten. Dette har sammenheng med innføringen av Samhandlingsreformen og at befolkningen blir eldre og har stadig flere komplekse, kroniske helsetilstander. Totalt øker omfanget og kompleksiteten på den helsehjelpen som behøves i hjemmet og det forventes at profesjonene som tilbyr helsetjenester kan tilby rask og persontilpasset helsehjelp. For å imøtekomme denne utviklingen etablerte HO i 2021 en virtuell helseavdeling (VHA) som har til hensikt å forebygge reinnleggelse på sykehus og institusjon, avlaste korttidsplasser, heve kompetansen i hjemmetjenesten og skape smidige overganger mellom tjenestenivå. VHA (i litteraturen kjent som "Virtual Ward") er?ikke en fysisk avdeling med inneliggende pasienter, men en måte å følge opp pasienter i hjemmet ved hjelp av teknologi, avstandsmonitorering og fysiske besøk. Det er begrenset litteratur knyttet til virtuelle helseavdelinger i en norsk kontekst og Bodø kommune vil systematisk dokumentere og skape ny kunnskap om: - Hvilke pasientgrupper synes særlig å egne seg til virtuell oppfølging i eget hjem? - Hvordan kan sykepleierens kliniske blikk utøves i en virtuell setting? - Hvordan opplever pasienter og pårørende å være mottaker av tjenesten? I prosjektet vil sykepleie som profesjon og utøver av helsehjelp særlig bli studert for å se hvordan det kliniske blikket utfordres og må utføres på nye måter når observasjonen av pasienten medieres av teknologi. Studien er oppbygget som en mixed methods-studie med deskriptiv statistikk, fenomenologiske dybdeintervju og feltobservasjon.
