Ettersom flere mennesker lever lenger, trenger vi flere helsetjenester, spesielt for akutte helseproblemer. Dette legger mye press på helsesystemet vårt, som inkluderer hjemmeomsorg, sykehjem, lokale klinikker og sykehus. For å sikre at vi kan ta vare på alle i fremtiden, må vi finne måter å hjelpe eldre mennesker med å holde seg sunne og unngå å trenge mye omsorg.
PAI-prosjektet jobber med dette problemet. Prosjektet bruker kunstig intelligens (KI) for å forutsi hvilken type omsorg eldre mennesker kan trenge ved å se på helsedata. Ved å oppdage helseproblemer tidlig, kan vi ta skritt for å forhindre at de blir verre. Dette kan bety at vi forhindrer fall i sykehjem eller endrer en persons medisinering på deres lokale klinikk. Dette bidrar til å unngå sykehusopphold, som kan være dyre og stressende.
Imidlertid er det vanskelig å forutsi helsebehov fordi dataene vi har kan variere mye. Det kan være forskjellig avhengig av hvor personen bor, og hvilke helseproblemer de har. For å håndtere dette, jobber PAI-prosjektet med mennesker som mottar omsorg og helsearbeidere for å utvikle KI-verktøy. Prosjektet tester disse verktøyene i virkelige situasjoner i Spania, Sverige og Norge.
Prosjektet bruker en blanding av metoder for å samle inn og analysere data. Dette inkluderer både tall (kvantitative data) og folks erfaringer og meninger (kvalitative data). Målet er å hjelpe eldre mennesker ved å gi dem riktig omsorg til rett tid, forhindre at skader og sykdommer blir verre. Det hjelper også helsearbeidere ved å sørge for at ressursene brukes effektivt og skaper et godt arbeidsmiljø. Løsningene de finner kan brukes på mange forskjellige steder.

As the population ages, the demand for healthcare services increases, including acute care. This puts a strain on healthcare resources, making efficient care management crucial at all levels - home, nursing homes, primary care, and hospitals.
The PAI project aims to address this challenge by using artificial intelligence (AI) to predict the care needs of older individuals. By analyzing patient monitoring data, the project can detect early signs of health or functional decline. This allows for preventive measures to be taken, reducing the need for costly acute care and aftercare. For example, hospitalizations can be prevented by avoiding falls in nursing homes or adjusting medication in primary care.
However, predicting the care needs of older patients is not straightforward. It involves dealing with varying data availability, quality, and potential interventions across different care levels and diagnoses. To tackle this, the PAI project uses a design science approach and a multiple case study design. It develops analytics in collaboration with care recipients and healthcare practitioners and implements them in real-life settings in Spain, Sweden, and Norway.
The project uses a mixed-methods strategy for data collection and analysis, combining quantitative and qualitative elements. This includes the thematic interpretative analysis of meeting notes, interviews, and observations, as well as the statistical longitudinal trend analysis of Key Performance Indicators in the respective healthcare systems.
The benefits of the PAI project are twofold. Older individuals receive adequate early care, preventing injuries and the worsening of diseases. Healthcare staff can use their resources more effectively, creating a sustainable and satisfying work environment. The outcomes of the project are scalable and can be applied in various contexts, making it a valuable contribution to the future of European healthcare.

Predicting Care Needs of Older Adults in the Healthcare System through AI-enabled Analysis of Patient-Monitoring Data

Limb wounds in horses are common, however healing is exceedingly slow and fraught with complications. Contrary to body wounds, limb wounds demonstrate only a small amount of wound contraction. The specialist procedure of applying split-thickness skin grafts to limb wounds stimulates wound contraction by unknown mechanisms. The objective of this project is to identify those mechanisms to develop a treatment strategy for equine limb wounds for use in general  practice. Limb wounds are severely impacting animal welfare due to an extended healing/treatment duration, and the risk of euthanasia due to a poor functional outcome. By stimulating wound contraction, healing time will be reduced, and the functional outcome improved, directly benefitting the welfare of wounded horse and reducing treatment cost for the owner. Aim: To identify dermal components stimulating wound contraction, and to develop this knowledge into future state-of-the-art treatment of equine limb wounds for use in general practice. Hypotheses: i) Application of dermal fractions to collagen gels seeded with equine fibroblasts will increase gel contraction in vitro; ii) Application of dermal fractions to experimental limb wounds in vivo, will increase wound contraction resulting in a faster healing time versus wounds treated by conventional methods. Methods: i) The contractile effect of different dermal components will be assessed in a collagen gel/ fibroblast model; ii) Identified dermal components will be tested in a standardized experimental limb wound model in vivo (n=5). Ultimately, results from this project are expected to improve wound treatment in horses, directly impacting animal welfare by reducing healing time and improving the functional outcome of wounded horses. This will also directly benefit horse owners and caretakers and making veterinarians able to offer better treatment solutions.

Bilingual Experiences, Cognitive Ageing and underlying Mechanisms

The goal of the Bilingual Experiences, Cognitive Ageing and underlying Mechanisms (BECAME) project is to examine the functional neuroplastic mechanisms underlying mitigatory effects of bilingual experience on age-related cognitive decline. While evidence is available for structural brain changes supporting bilingualism-induced effects on cognitive aging (CA), functional mechanisms underlying such effects are not yet well understood. BECAME aims to fill this gap by investigating the trajectory of bilingualism-induced functional neuroplasticity across the adult lifespan (ages 18-85), crucially adopting an individual differences approach related to measurable degree of bilingual engagement. Importantly, although such benefits are thought to be rooted early in the lifespan, evidence for the timeframe and conditions of their early emergence is scarce. The choice of including the whole adult lifespan allows us to investigate these mechanisms and their emergence more directly from a lifespan perspective. I combine behavioral testing with brain recordings via electroencephalography (EEG). EEG is widely used in cognitive neuroscience to assess brain activity and the degree of communication between brain regions (functional connectivity) in response to stimuli and at rest. Combining behavioral and EEG modalities will allow me to test bilingualism’s effect on functional connectivity, as well as its consequences for functional efficiency, the minimum required neural activation to successfully perform a task. This will enable a detailed, evidence-based account of the protective effects observed in older-aged bilinguals, and how they are built from earlier life stages. By illuminating the relationship between multilingualism and resulting neurocognitive adaptations, BECAME will enable to capitalize on it as a health initiative, with manifold societal implications and the potential to be translated into actionable policies exploiting multilingualism to ameliorate effects of CA.

Wheelchair users (WCU) are three to five times more inactive than the general population, leading to a high risk of developing preventable secondary conditions. Although physical activity (PA) guidelines have been specifically developed for WCU, we lack systematic approaches to implement them in practice, as well as valid algorithms to monitor PA change in WCU through sensor-based solutions. Accordingly, the overall scientific aim of the WHeelchair Activity Monitoring project (WHAM) is to advance PA research and practice in WCU by developing valid digital markers for monitoring of and providing feedback on PA, paving the way for implementation and intervention. I will accomplish this through combining the complementary research fields of 1) behavioural intervention studies focusing on increasing PA in WCU (core competence University of British Columbia, UBC; outgoing phase) and 2) sensor-based PA monitoring (core competence Norwegian University of Science and Technology, NTNU; returning phase – consolidated by non-academic placement at Maastricht Instruments, which develop research prototypes including PA monitors). WHAM’s scientific aim is achieved through three objectives: 1) Measure the effect of a pragmatic exercise intervention trial on self-reported physical activity levels and cardio-respiratory fitness, 2) Develop valid algorithms for tracking PA markers in WCU, and 3) Co-create a framework that outlines how PA can be conjointly monitored and provided feedback on in WCU. This fellowship will enable me to become established as an internationally recognized researcher in the field of “physical activity and disability”, while bringing together ongoing national disjointed research activities in this field. Altogether, my overall ambition is to contribute to reducing inequalities in daily-life participation among WCU compared to the rest of the population.

WHeelchair Activity Monitoring project (WHAM)

Hovedmålet med kreftbehandling er å eliminere kreftceller og dette er tradisjonelt oppnådd ved kirurgi, bruk av giftige medikamenter (cellegift) og stråling. En ny og betydningsfull tilnærming til kreftbehandling kalt immunterapi, har derimot fått mye oppmerksomhet det siste tiåret; Her forsøker leger og forskere å utnytte immunsystemet vårt for å målrettet  ødelegge kreftceller. En nøkkelaktør i denne banebrytende tilnærmingen er en gruppe «drapsmenn» i immunsystemet kjent som cytotoxiske T-lymfocytter (CTL-er). Disse cellene har en bemerkelsesverdig evne til å oppdage kreftceller ved å gjenkjenne unormale molekyler på overflaten av kreftcellene - molekyler som friske celler ikke har. Når CTL-er gjenkjenner kreftcellene ødelegger de dem ved å frigjøre giftige molekyler som initierer kreftcellenes selvdestruksjon. 
 
Selv om betydelige innsats har blitt dedikert til å forstå hvordan CTL-er aktiveres og hvordan de gjenkjenner kreftceller, vet vi mindre om de påfølgende hendelsene inne i kreftcellene som fører til deres død. Fra før vet vi at CTL-er kan initiere to forskjellige typer celledød i kreft: den første, kalt apoptose, er en stille og ordnet prosess der kreftcellene krymper og brytes opp uten å varsle immunsystemet; den andre, kalt pyroptose, er en mer dramatisk prosess som involverer dannelse av åpninger i cellemembranen, noe som fører til at cellene svulmer opp og sprekker. Under denne cellebristen blir innholdet inne i cellene frigitt i omgivelsene, og dette kan fungere som 'faretegn' som kan stimulere en sterkere immunrespons mot kreftcellene. Betydningen av denne cellebristen for å forsterke anti-tumorresponsene vil bli undersøkt i dette prosjektet. 
 
Videre er vi fascinert av muligheten for å avdekke ytterligere, ennå ukjente, former for celledød indusert av CTL-er. Utforsking av disse dødsrelaterte prosessene i kreftcellene kan potensielt øke effektiviteten av immunterapi og bidra betydelig i den pågående kampen mot kreft.

The primary goal of most cancer therapies is to induce cancer cell death. Immunotherapy relies on the cytotoxic activity of lymphocytes such as natural killer cells (NKs) and cytotoxic T lymphocytes (CTLs) to achieve this. While a significant amount of research in immunotherapy focuses on the events within cytotoxic lymphocytes or at the interface between cytotoxic lymphocytes and cancer cells, less is known about the molecular mechanisms that orchestrate cancer cell death, particularly in the case of CTL-mediated death.
CTLs are known to induce two types of cell death in cancer: non-inflammatory apoptosis and proinflammatory pyroptosis. Pyroptotic cancer cells release proinflammatory signals that attract immune cells to the tumor site, thereby enhancing the anti-tumor response. Cell lysis is a common outcome of pyroptotic cell death. It is unclear whether lysis can be decoupled from pyroptosis in cancer cells and whether the proinflammatory properties of pyroptotic cancer cell death depend on lysis. Moreover, it remains unknown whether CTLs can induce other forms of cell death in cancer, apart from apoptosis and pyroptosis.
Studying the mechanisms of CTL-mediated cancer cell death is challenging because CTLs selectively target cells expressing specific antigens. However, given the critical role of CTLs in immunotherapy, it is essential to deepen our understanding of CTL-induced cancer cell death.
This project aims to develop novel methodologies to investigate the death mechanisms that are initiated by CTLs. In addition, we will identify new pathways and mediators involved in CTL-induced cancer cell death. Furthermore, we will assess the contribution of proinflammatory cell death and lysis in promoting anti-tumor responses in vivo. Overall, this research will improve our knowledge of the molecular mechanisms of CTL-mediated cancer cell death and potentially reveal novel targets to enhance the efficacy of cancer immunotherapy.

En av de vanligste, men minst studerte, utfordringene med å bli eldre er problemer med å kommunisere. Mange eldre sliter spesielt med pragmatiske ferdigheter, som å forstå undertekst, eller tilpasse det de sier til personen de snakker med. 

La oss si at en lege må gi en alvorlig beskjed til en pasient. For å gjøre det litt lettere å takle, bruker legen vagere språk, som "kanskje" i stedet for "sannsynligvis". Mens yngre voksne ofte tar hensyn til høflighetsstrategier i måten de tolker det som blir sagt på, har eldre ofte vanskelig for å forstå når noen snakker indirekte. Mange eldre voksne har vanskelig for å forstå ikke-bokstavelig språk og kan misforstå alvorlighetsgraden av diagnosen. Dette kan påvirke beslutningen deres om behandling, noe som kan føre til utilsiktede konsekvenser. 

Siden god kommunikasjon er viktig både i viktige medisinske samtaler og i hverdagen, er det viktig at vi finner ut hva som gjør at eldre kan ha vansker med å forstå og bli forstått. RESILIENCE skal hjelpe til med denne utfordringen ved å samle data om praktiske, kognitive og språklige ferdigheter hos voksne i alderen 18-90 år over tid. Dette vil gi oss en stor mengde data om pragmatiske, kognitive og språk ferdigheter og gjøre det tilgjengelig for forskning. Med denne dataen som grunnlag har vi følgende mål: 

• Forstå hvordan praktiske evner endrer seg generelt gjennom voksenlivet. 
• Finne ut hvilke mekanismer som påvirker pragmatiske evner på forskjellige tidspunkter i livet.
 • Utvikle en teoretisk modell som forklarer hvordan pragmatiske ferdigheter endrer seg med alderen.
 • Lage algoritmer som kan forutsi hvordan pragmatiske evner vil utvikle seg for enkeltpersoner i årene som kommer. 
• Utvikle og teste et verktøy som kan måle pragmatisk helse (både pragmatiske ferdigheter og deres underliggende mekanismer), noe som kan åpne  for å iverksette tiltak. 

Dette vil gi oss muligheten til å forstå bedre hvordan folk kan utvikle pragmatisk resiliens gjennom livsløpet.

A cornerstone of human communication is our reliance on pragmatic skills to guide linguistic interactions (e.g., reasoning about others' mental states to bridge the gap between what is said and intended in conversation). However these skills are highly sensitive to age-related decline, leading to miscommunication and uninformed decision making in the older adult population. This raises an important question, one which has puzzled linguistics and psychologists for decades: if pragmatic competence is fundamental to communication, why do individuals vary so much in this capacity? 

Traditionally, psychologists have approached pragmatic competence from a cognitive angle, reasoning that individual differences in executive functions (i.e., the cognitive skills associated with goal-directed actions) are what underlie effective pragmatic behavior. In a similar vein, language scientists have taken a linguistic approach, reasoning that one’s language background (e.g., the frequency with which individuals shift between languages) shapes pragmatic skills by enhancing sensitivity to the communicative context. This focus on either cognitive or linguistic influences has resulted in two expansive yet disparate areas of research.

RESILIENCE will unite these disciplines to pursue a longstanding debate on what underlies pragmatic resilience and decline using a large-scale, interdisciplinary and longitudinal approach. By gathering data on adults' (ages 18-90) pragmatic, cognitive and language skills over multiple time points, we will develop a comprehensive theoretical model that explains how cognitive and linguistic systems interact to shape pragmatic competence as we age, and create algorithms that predict the trajectory of an individual’s pragmatic abilities in upcoming years. Based on these results we will design a multidimensional tool that detects the earliest signs of pragmatic deficits, leading to future interventions, allowing us to uncover the paths to pragmatic resilience.

Uncovering the paths to pragmatic resilience

Avansert nedbrytbar nanocellulose-matrix for stamcelle-differensiering og tilheling av brannskader.

Cellu4Heal-prosjektet går ut på å utvikle en ny, avansert behandling for alvorlige brannskader. Siden 1970-tallet har behandlingen av brannskader i stor grad basert seg på kirurgi og hudtransplantasjoner. De siste tiårene har forbedret behandling ført til færre dødsfall, men det er fortsatt et stort behov for mer effektive metoder for å bedre sårtilhelingen og gro ny hud. I denne sammenhengen har behandlinger med mesenkymale stamceller (MSC) vist lovende resultater. Tidlige studier viser at MSC kan hjelpe sår med å gro raskere og forbedre arrkvaliteten over tid. Imidlertid gjenstår utfordringen med å levere disse cellene til såret på en god og sikker måte.

Cellu4Heal-prosjektet utvikler en biologisk nedbrytbar gel, kalt Cellu4Heal matrix, laget av et naturlig materiale (nanocellulose). Denne gelen er designet for å bli trygt absorbert av kroppen, og vil bidra til å fremskynde sårtilhelingen, redusere betennelse og motvirke arrdannelse. Med gelen vil man kunne levere MSC direkte til såret, noe som kan føre til raskere bedring og bedre langtidsresultater for pasientene. Gjennom prosjektet vil det også bli utviklet et smart spraysystem for å påføre behandlingen på sår på en enkel og effektiv måte. Denne innovative tilnærmingen kan i stor grad redusere risikoen og komplikasjonene knyttet til alvorlige brannskader, forbedre overlevelsesraten og betydelig øke pasientenes livskvalitet.

Prosjektet er et samarbeid mellom Universitetet i Bergen (UiB, Norge), Haukeland Universitetssjukehus (HUS, Norge), RISE PFI (Norge), National University of Science and Technology Politehnica Bucharest (UPB, Romania), Universitetet i Helsinki (UH, Finland) og en industriell partner (Alginor Biorefinery AS (ABR AS), Norge).

Since the 1970s, large burn wounds have been treated with essentially the same strategies including surgical excision and grafting. Although later improvements in intensive care have led to decreased mortality, there is a great need for further advancements in wound healing and skin regeneration. In recent years, the use of mesenchymal stem cells (MSC) in the treatment of burns has gained increasing attention. Preclinical studies have indicated both local improvements in burn wound healing and long-term improvements in scar quality and contractures. However, there is a lack of a functional matrix for accurate dosing and delivery of MSC to the wound, and the optimal clinical administration system for MSC therapy is yet to be developed. Our project aims to develop and validate an advanced hydrogel matrix based on nanocellulose (CNF) (Cellu4Heal matrix) innovatively engineered with biodegradability and antibacterial properties that specifically meets the requirements of MSC delivery and therapeutically modifies the burn wound environment. The Cellu4Heal matrix can be utilized for accurate dose-controlled administration of MSC therapy to facilitate and promote skin regeneration of severe burn injuries. The Cellu4Heal project will develop an advanced degradable nanocellulose-based scaffolding material that is biocompatible and will add the novelty of built in properties of biodegradability to avoid remaining foreign material in the body beyond the period needed for the cell therapy to exert its sought effects of supporting skin regeneration. In addition, a novel intelligent spray-on delivery system will be developed for controlled administration of the Cellu4Heal matrix on the wounds. 
The project is a collaboration between University of Bergen (UiB, Norway), RISE PFI (Norway), National University of Science and Technology Politehnica Bucharest (UPB, Romania), University of Helsinki (UH, Finland) and an industrial partner (Alginor Biorefinery AS (ABR AS), Norway).

I Europa finnes et betydelig antall voksne som har sammensatte og langvarige helsetjenestebehov (SLB), og mange håndterer flere slike tilstander (Multi-SLB). På grunn av tjenestenes reaktive natur, mangel på koordinering, begrensede tilgjengelighet og sykdomssentrerte fokus, er det ufordrende for de tradisjonelle tjenestene å møte denne gruppens behov.

Digital-CACTUS-prosjektet undersøker potensialet for digital støttede pasientforløp, som kombinerer tradisjonelle og digitale helsetjenester som videokonsultasjoner, avstandsoppfølging og digitale behandlinger. Denne tilnærmingen tar sikte på å gjøre helsetjenestene mer proaktive, personlige og tilgjengelige.

Prosjektet har tre hovedmål:

* Utvikle et verktøy for hvor pasienten rapporterer behov for digital støtte : Dette verktøyet vil identifisere hvor digitale løsninger kan forbedre omsorgen ved å peke ut systemfeil og foreslå passende digitale inngrep.
* Evaluere effekten av digitalt støttede pasientforløp: Kartlegging av bruken av digitale verktøy over hele Europa for å vurdere deres effektivitet og aksept i ulike helsetjenester.

Digital-CACTUS kombinerer ekspertise innen Multi-SLB, digital medisin og pasientsentrert omsorg, med mål om å gi helsemyndighetene evidensbaserte anbefalinger. Dette sikrer at digital helsetjeneste bevarer grunnleggende menneskelige verdier og styrker tjenestenes bærekraft.

In European countries 25% have multiple long-term chronic conditions (MLTCs). Persons with MLTC represent 2/3ds or more of health care spending, because care systems are designed to handle single disease patients. Innovation of care, in terms of blended care models that augment traditional face-to-face care with technology is key to sustainability and continued quality of care. Blended care tools form a continuum from video consultation, asynchronous message exchanges, to remote patient monitoring and automated just-in-time interventions. Blended care models can support person-centered, proactive care, reduce the need for face-to-face visits and tailor care to the patients’ lives. However, poor implementation of blended care may disrupt workflows, cause digital exclusion or alter patient-clinician relationships.

Our multidisciplinary research program aims at uncovering the potential gains and side effects of blended care at both patient and health system levels with a comprehensive research program with three work packages (WP).

WP1. How will the digital transformation change patient–clinician relationships?  The answer comes from an international survey eliciting patient and clinicians experiences. 

WP2. Which care-challenges could have been better supported by digital tools? Patients will recreate their care journey and indicate 1) “failures in care” and 2) whether blended care tools could have avoided or alleviated these failures.

WP3. Mapping the potential impact of blended care in Europe through a large international survey of patients using the tool from WP2. 

We are five renowned teams in MTLCs, digital medicine and patient centered care, as well as a private partner which will translate research results into actionable tools.
Our project provide decision makers with evidence on how technological tools should be implemented to optimize the delivery of health and care services while preserving the humanistic values of patient-clinician relationships.

Cancer-related cachexia is connected with significant weight and muscle loss is observed in 50% of the cases and is a deadly condition associated with chemotherapy. No standard treatment for this condition is presently available. In these patients, a high level of proinflammatory cytokines is observed.  

The yearly number of cancer deaths worldwide is around 10 million, of which about 30% can be related to cachexia. In 2020, EU27 had an incidence of new cancer cases of 2,7 million with approximately 1,3 million cancer-related deaths, of which as many as 400 thousand – about 30 % – may potentially be linked to cachexia. There are currently no effective remedies for cancer-induced cachexia on the market.

Our innovation, AndoSan, is an extract based on 3 mushrooms. AndoSan is produced through a unique proprietary process leading to unique chemical composition. Even though the immunomodulating effects of AndoSan have been studied extensively, its potential for cancer cachexia has just recently become evident. A surprising 50 % to 90 % reduction in levels of proinflammatory cytokines such as TNF-a and IL-6 observed in recently conducted cell assay combined with post-analysis of clinical pilot trials in humans focused on other indications strongly suggests a potentially game-changing role for AndoSan as a novel treatment of cancer cachexia.
