Phenotyping of adolescents with bipolar disorder and with schizophrenia in a longitudinal study

The currently used categorical ICD-10 and DSM-IV diagnoses play an important part in many areas of clinical psychiatry and psychiatric research. However, recent focus in genetic research on the severe mental disorders increases the need for theoretical co nstructs that are more precise descriptions of the symptoms or phentotypes. The overall objective of the proposed post-doc application is an endophenotyping by detailed clinical characterization and by multimodal MR imaging of young patients with schizoph renia (SZ) and bipolar disorder (BP) aged 13 through 20 years at the time of inclusion. The proposed study has a longitudinal design with an examination of 30 patients in each diagnostic group (early onset SZ, BP and controls) at baseline, after two, five and ten years. Specifically, we aim to test two hypotheses: a) Frequency and intensity of auditory hallucinations in both diagnostic groups will be studied in an experimental design with dichotic listening to speech sounds. It is predicted that failure of perceiving external speech sound stimuli will correlate with frequency of auditory hallucinations. b) It is predicted that patients in both diagnostic categories will be less successful than control subjects in cognitively modulating their affect rea ctions during a combined working memory and facial recognition fMRI-paradigm. Clinical characterization will include a ?best estimate? consensus based on a structured diagnostic interview and symptomlists. Consensus will be calculated from estimates by two experienced clinical psychiatrists. The neuropsychological testing will assess the general cognitive functioning and the patients? ability of affect regulation. Multimodal MR brain imaging will include structural MRI, functional MR, Diffusion Tensor I maging and MR spectroscopy. The longitudinal approach of the project will allow for a characterization of dynamic developments that are typical for these two diagnostic categories.
