Crisis Resolution and Home Treatment in Community Mental Health Service: Development, Practice, Experiences, and Outcomes

One major development in the provision of adult mental health services during the last decade is a shift from in-patient services to community care. Various models of community mental health care have been established with an aim to minimize hospitalizati on and maximize acute care and rehabilitation in the context of an individual's immediate social environment. One such model is Crisis Intervention and Home Treatment (CR/HT) services. In Norway, a mandate calls for an establishment of CR/HT teams in all of its 78 DPS units by 2008. Although the experiences of CR/HT services have been discussed in the literature, there is still a lack of in-depth knowledge about various aspects of CR/HT, including implementation processes, service-user responses, and link age to outcomes. We thus aim to advance the knowledge through three studies from three different orientations. Study 1 will focus on the implementation of a CR/HT team in a local Mental Health Service unit, examining processes and dynamics that play a cri tical function in the implementation of the team and its practice. It will be done through a prospective case study within an action research perspective, involving members of a local CR/HT team. Study 2 will focus on the experiences of service-users, esp ecially those with psychosis, in relation to how they experience crises and experience them in the context of everyday life, and how they experience mental health services, especially CR/HT. It will be done in a qualitative descriptive design, applying fo cus-group and phenomenological approaches. Study #3 will focus on the impact of CR/HT teams on macro-level outcomes across the mental health service areas nationally and more specifically in the Health South Region of Norway. It will be done in a quantita tive descriptive design utilizing survey and secondary data. The results from these studies can be applied in developing benchmarks for implementation and evaluation of CR/HT teams at local and aggregate levels.
