Periodontal disease is a family of bacterial infections characterized by the destruction of periodontal supporting tissues. The bacterial flora at the diseased sites is complex, counting more than 500 different species in the subgingival dental plaque. Th ree main hypotheses exist for the infectious nature of periodontal disease; The non-specific plaque hypothesis, The specific plaque-hypothesis, The ecological plaque hypothesis and The hypothesis of bacterial complexes. The standard treatment of periodon titis has been to mechanically reduce the number of bacteria present by scaling and root planing (SRP), thereby reducing the bacterial challenge in the sites showing disease progression. However, a high recurrence rate is reported after this kind of treat ment. Adding antibiotics to this treatment, which has been suggested by some researchers, has not increased the long term success-rate of the therapy. A new treatment modality has been added to the arsenal as full mouth disinfection (FDIS) has shown posi tive results in a series of clinical studies, although the success failed to reproduce in other studies. However, FDIS is logically potentiating and increasing the reduction of microbial load when SRP, explaining its short-time success. Thus FDIS may be an extra additional factor in treating periodontal diseases with SRP and Antibiotics since it reduces the bacterial challange beyond that possible of SRP. It is possible to combine the SRP, antibiotic treatment and Full Mouth disinfection in one, short a nd resource saving treatment strategy. Thus, the present study is designed to compare the SRP, SRP + traditional antibiotics, and SRP combined with antibiotics and FDIS and to follow the treatment result during a 5 year- follow up. If this novel strategy prov to be more-, or as successful as traditional treatment it may result in changes in the public spending policy (through NAV) and the guidelines provided with this refund policy.
