Biomechanical characteristics of patients with hip osteoarthritis with mild to moderate pain: A prospective, randomized clinical trial

This project funding proposal concerns a personal postdoctoral fellowship within the area of clinical research. The project will investigate the effect of exercise therapy and patient education compared to patient education alone on lower extremity biomec hanics during gait, sit-to-stand, and one-legged standing balance from a randomized controlled trial (RCT) including patients with mild to moderate hip OA. To our knowledge, the project will be the first RCT to assess biomechanical data for hip OA patient s at this level of severity. Osteoarthritis (OA) is already within one of the ten burden diseases in high income countries; and because of the rapid increase in the percentage of people older than 50 years OA is a growing health problem. As a conse quence, OA poses considerable demands on the community and health care system with regard to treatment and sick leave. In order to meet these challenges it is important to increase current knowledge on the factors contributing to development of OA severit y, and how these factors potentially may be targeted through treatment and rehabilitation. Most of the existing evidence on interventions in patients with OA is, however, reported from clinical trials on patients with knee OA. RCTs on patients with hip OA are sparse. As a consequence, knowledge on treatment modalities for patients with hip OA is still lacking. The results of the study could have important implications for the understanding on how changes in lower extremity biomechanics may be related to the level of hip OA severity and disease development. The RCT design allows investigations into whether exercise therapy may decelerate destructive movement alterations, and has consequently an inherent potential to improve our ability to develop targete d treatment interventions with the long-term intention of slowing down the progression of the disease; and thus postpone the need for total hip replacement surgery.
