European Social Communication Autism Trial

Proposal to H2020, Call: PHC-18-2015: Autism spectrum disorder (ASD) is a common and costly paediatric condition. Psychological interventions are the primary treatment. However, current intervention provision across Europe is highly variable and inadequate. We propose to undertake a randomized controlled trial (RCT) in 5 countries to test the clinical and cost-effectiveness of a short-term, moderate-cost parent- and educator-delivered treatment (JASPER) that has promising evidence for efficacy. ASD affects 1% of children and is associated with high morbidity and cost to the individual, their family and society, with estimated lifetime costs of 1.1 to 1.8 million euro per person. Widespread uptake of effective low-cost interventions in young childhood may help avert some of these costs and increase productivity across Europe, not only in childhood but also as these children grow into adults. We will use multiple cutting-edge technologies (EEG, eye-tracking, genetics) to test whether biomarkers can be used as stratification factors or surrogate outcomes for treatment responsiveness. We will test the utility of novel health technology assessments, including automated capture of behaviour, portable eye-tracking technology and web-based sharing of video material, to be used in future trials and clinical services to efficiently measure treatment response and outcomes. We will identify whether intervention "normalizes" brain and cognitive function or works via alternative compensatory neurodevelopmental processes. We will assess the acceptability of the intervention and the service characteristics that enable the intervention to be successfully adopted within different communities, so that it can be widely implemented across Europe. We will deliver an intervention programme that will transform early autism treatment provision across Europe and provide significant benefit to the 790,000 of children in Europe affected by ASD, their families and wider European society
