Structure-Function Studies and Inhibitor Discovery Targeting the Proton-Gated Urea Channel and the Cytoplasmic Urease from H. Pylori

Helicobacter pylori-infeksjon i magens slimhinner er fortsatt et verdensomspennende problem og kan resultere i både magesår og magekreft. Uten aktive inngrep vil minst 20 % av befolkningen i utviklede land fortsette å bli smittet av dette gastriske patogenet. Utryddelse av denne organismen vil kunne bidra til forebygging av sykdom. Nåværende fjerning av bakterien krever en trippel-behandling: en proton-pumpe inhibitor samt to typer antibiotika som skal tas to ganger daglig i 10-14 dager. Resistensen mot enten klaritromycin eller metronidazol er alt på over 25 % og stiger, og det finnes heller ingen effektiv monoterapi. Gastrisk infeksjon av H. Pylori avhenger av ekspresjon av en bakteriell ureakanal (HpUrel) og det cytoplasmatiske enzymet urease (HpUreAB) som er unik for dette patogenet. HpUrel er nødvendig for rask tilgang av urea til den intra-bakterielle ureasen. Dette enzymet er essensielt for at periplasma skal opprettholde pH 6,1 i det sure miljøet i magen (så lavt som pH 1), og slik muliggjøre kolonisering av magen. Kanalen er et membranprotein bestående av 195 aminosyrer i seks trans-membrane segmenter. Vi har tidligere fastslått den tredimensjonale strukturen til denne kanalen (3,2 Å) og til det nedstrøms 1,1 MDa urease-komplekset (2 Å), og har i dette prosjektet utnyttet den kunnskapen for å øke vår forståelse av mekanismen kanalen bruker for å transportere urea samt å identifisere bindingsseter for inhibitorer, hvorav noen alt er oppdaget. Vi håper at inhibering av denne kanalen eller av urea-komplekset på sikt resulterer i spesifikk og effektiv monobehandling for utryddelse av denne organismen og at det vil innlede en ny æra der man tester og behandler fremfor bare å behandle pasienter med symptomer. Dette vil gi en forebyggende tilnærming til alvorlige sykdommer i den øvre delen av magen, særlig magekreft som forårsaker rundt 750 000 dødsfall årlig. Vi har brukt CryoEM-teknologi for strukturanalyse, og har bestemt cryo-EM-strukturen til Helicobacter pylori-urease med en inhibitor i det aktive stedet ved 2,0 Å oppløsning. Vi har videreutviklet en enkel metode for å teste urease ved bruk av E. coli-celler, og identifisert flere mulige ureasehemmere som kan penetrere cytoplasmaet og virke ved lav pH ved bruk av E. coli-uttrykkende H. pylori-urease-operon.

Infection of the gastric mucosa by Helicobacter pylori remains a worldwide problem and contributes to peptic ulcer disease and gastric cancer. Without active intervention, at least 20% of the population of developed countries will continue to be infected by this gastric pathogen. Eradication of the organism would contribute to prevention of disease. Current eradication requires triple therapy: a proton-pump inhibitor and two antibiotics given twice a day for 10 to 14 days. Resistance to either clarithromycin or metronidazole is >25% and rising and no monotherapy is effective. Gastric infection by H. pylori depends on the expression of a bacterial urea channel (HpUreI) and a cytoplasmic urease (HpUreAB) unique to this pathogen. HpUreI is a proton-gated urea channel necessary for rapid access of urea to the intra-bacterial urease, essential for maintaining the periplasm at pH 6.1 in the acidic environment of the stomach (as low as pH 1), thus enabling colonization of the stomach. The channel is a membrane protein with 195 residues in six transmembrane segments. Previously, we determined the 3-dimensional structure of this channel (3.2 Å) and of the downstream 1.1 MDa urease complex (1.98 Å), and we now propose to exploit that knowledge to increase our understanding of the proton-gating mechanism and to identify binding modes of inhibitors, some of which have been discovered already. Inhibition of the cytoplasmic urease complex is expected to result in specific and effective monotherapy for eradication of the organism and usher in an era of test and treat, rather than only treating symptomatic patients. This would provide a preventive approach to serious upper gastro-intestinal diseases, particularly stomach cancer, which causes approximately 750,000 deaths annually.
