Prosjektide 1: Telemonitoring at home CONCEPT: The idea is to implement a nurse-led integrated model of care to support complex patients as they move from hospital back to their home/community. We will need to work on defining an intervention that is generalizable enough and that considers the variability in existing models of care across partnering countries. Central to this grant call is that our proposed nurse-led model of care needs to include a technology component that supports patient transition in care. Next step - Partnermøte i Dublin januar 2020 Prosjektide 2: Providing digitalised prevention and rehabilitation support for ageing people in smart living environments CONCEPT: Few structures in current care systems across the EU and Canada systematically support older citizens to prevent new care needs from occurring, and to regain independence from care services. We are talking about true person-centeredness in a systemic context where disease, frailty, pre-frailty and health are transformed into free, active and independent living. Based on experience and insights, our concept is about embedding validated digital health solutions and tools into smart living environments that support ageing people to continue to shape their lives in ways that are self-determined, relatively active and connected to their communities and social networks.
