Implementing a user involved education- and health system interactive task-shifting approach for child mental health promotion in Uganda

Hvordan kan flere barn få hjelp med psykiske helseproblemer i et land med 20 millioner barn og kun 7 barnepsykologer? Landet det er snakk om er Uganda. Til tross for økonomisk vekst, lever fortsatt 42% under fattigdomsgrensen. Barn som vokser opp i fattigdom har en økt sårbarhet for både fysiske og psykiske helseplager. Mange opplever også vold og traumer. I tillegg til vold i hjemmet, utsettes over halvparten av barna i Uganda for fysisk vold fra lærere. Hvordan kan vi forebygge vold og bidra til at barn med psykiske helseproblemer får hjelp, når behovet er så stort, og ressursene så knappe? I dette forskningsprosjektet vil lærere få opplæring i psykisk helse og enkle forebyggende tiltak med mål om å fremme og psykososial oppfølging på skolen, forebygge skolevold og redusere stigma knyttet til psykisk helse. Gjennom å utvikle og integrere systemer for samarbeid mellom utdannings- og helsesektoren, er målet at lærere kan gi psykososial oppfølging samt fange opp og henvise barn med alvorlig problematikk til første- eller andrelinjetjenesten. Forskerne vil samle inn data om implementeringsutfall og barnas utsatthet for vold og mulighet til å få hjelp før og etter implementering av the intervention TREAT INTERACT, en lærerrettet intervensjon utviklet basert på Verdens Helseorganisasjons (WHO) «Mental Health Gap Action Programme Intervention Guide» (mhGAP-IG). Målet med prosjektet er firedelt: 1) Utvikle en skoleversjon (s) av mhGAP-IG for lærere i grunnskolen (TREAT INTERACT) 2) Implementere og evaluere effekten av mhGAP-IGs 3) Opplæring av ansatte i skolen og helsetjenesten i henholdsvis TREAT INTERACT og mhGAP-IG og implementere og evaluere intra- og intersektorielt samarbeid 4) Utvikle kunnskapsbaserte implementeringsstrategier for å skalere opp TREAT INTERACT Gjennom å identifisere effektive implementeringsstrategier for hvordan TREAT INTERACT kan oppskaleres og vedlikeholdes, vil studien kunne bidra med kunnskap om hvordan psykiske helseprogrammer kan implementeres på en slik måte at de styrker tjenestene over tid når behovet er stort og ressursene knappe. Prosjektet gjennomføres av Nasjonalt kunnskapssenter om vold og traumatisk stress, Makerere University School of Public Health, Universitetet i Bergen, Folkehelseinstituttet og Norges teknisk-naturvitenskapelige universitet.

Uganda is recognizing mental problems among children as a major public health challenge. However, mental health services are facing lack of resources, stigmatizing attitudes, and a lack of help-seeking and systems for referrals. A task-shifting approach is highlighted as necessary to archive universal health coverage. The primary school attendance in Uganda is close to 100% for the first grades in primary school and constitute as such a unique opportunity for detection. The aim of the TREAT INTERACT project is to develop, implement and evaluate the impact of a novel, inter-sectorial TREATment INTERACTive program to identify, prevent and treat mental health problems in children and adolescents in Uganda through a user involved task-shifting adaptation and implementation of the WHO´s mhGAP-IG (Mental Health Gap Action Programme, Intervention Guide) among primary school staff. The overall goal is to generate knowledge on task-shifting implementation strategies to strengthen the inter-sectorial collaboration enhancing communication and referral strategies between the child’s school, community, and the health system. The TREAT INTERACT project group from the Norwegian Center for Violence and Traumatic Stress Studies, the University of Bergen and Norwegian University of Science and Technology in Norway, and the University of Makerere in Uganda, have designed a work programme consisting of 4 scientific work-packages (WP). In WP1 we will use intervention and implementation mapping and user-involvement in the adaptation of mhGAP’s child and adolescent mental health module for the primary school focusing on child development, behavioural- and emotional mental health. WP2 will test out the implementation and intersectoral communication strategies using a stepped-wedge design. WP3 will implement the mhGAP-IG school module together with a referral and supervision system and school communication platform. In WP4 we will assess sustainability and integration perspectives.
