Early identification and prevention of reduced oral Health in elderly reciving homecare services in Hedmark county.

The population of the elderly is increasing rapidly all over the world, and it is estimated that in Norway the population of people aged 65 and above will be 1,4 million in 2040. Today this population is approximately 840 000. More people live longer, both with and without diseases, and they often retain their natural teeth throughout their lifetime. Users of homecare services have rights to receive free public dental service. But KOSTRA-statistics based on numbers from all municipal in Norway, shows that it is great variation in terms of who receives the free dental service, and only one of four of them was examined/treated in 2015. It is a general conception that the oral health of the elderly in the homecare service, is a neglected area. This project will make it easier for the employees in the homecare service to identify the elderly with reduced oral health. The consequences of bad oral health can lead to serious disease, massive pain, and affect nutrition and psycho-social aspect in a negative way, such as the ability to talk and eat, and being social. We need a thorough survey of the elderly in the homecare service, and their perception of own oral health and life quality, and this project will gather this. Prevention of oral diseases and problems is of great importance, and can spare the patient for unnecessary pain and surgery, and furthermore, spare the society for unnecessary treatment costs. Multiple conditions associated with ageing and cognitive impairment can make the maintenance of own oral hygiene impossible. These patients need to receive a routine follow-up. Effective routines and adequate daily mouth care are, in addition to regular examinations, crucial in order to identify early oral problems and diseases. This does not exist today, but this project will contribute to establish routines and training methods for the home care service employees, and the public dental service.

Eldrebølgen og behovet for ulike helse- og omsorgstjenester samt tannhelsetjenester er et økende faktum. Flere eldre beholder egne tenner livet ut, ofte med komplekse tannerstatningsløsninger som medfører nye, økte og mer teknisk krevende behandlingsbehov. Erfaringer viser at mange eldre i hjemmetjenesten har god oral helse, spesielt når de selv er nye brukere og relativt friske. Likevel kan den orale helsen forfalle raskt, spesielt ved sykdom. Pleiepersonell trenger kunnskap nok til å registrere når den pleietrengende ikke lenger klarer å ivareta eget munnstell, og til å sette inn tiltak på munnstell ved behov. Forebyggende tiltak er viktige for å kunne forhindre forfall av oral helse, men det er per i dag mangel på kunnskap om hvilke tiltak som er de beste og mest kostnadseffektive for at eldre skal kunne opprettholde en god oral helse livet ut. Overordnet mål for prosjektet er å legge til rette for et bærekraftig, helhetlig tjenestetilbud og integrerte pasient- og brukerforløp for eldre i hjemmetjenesten. Ved å karakterisere behandlingsbehov, pasientbehov og samhandling mellom tjenestene, vil vi utvikle evidensbaserte tiltak for (tidlig) identifisering av redusert oral helse hos denne pasientgruppen. For å oppnå dette, skal følgende delstudier gjennomføres: 1: Kartlegge oral helse, behandlingsbehov og behov for hjelp med munnhygiene hos eldre i hjemmebasert omsorg for å videre identifisere hvilke omsorgstiltak som er de mest egnede for denne brukergruppen. 2: Kartlegge eldres perspektiv på sin orale helse og oral helse-relatert livskvalitet. 3: Gjennomføre kvalitative intervjuer med pleierne i hjemmetjenesten om kunnskap og holdninger om oral helse, for å kunne utvikle effektive rutiner og opplæringsmetoder for ansatte i hjemmetjenesten.
