Hva gir trygge overganger i interkommunal rehabilitering og hvilke metoder sikrer samhandling på tvers av helsetjenestenivåene?

The rehabilitation process for persons suffering from serious illness or injury commences in the hospital and care transfers to the care team within their own municipality. Historically services to this client group within the community have been compromised by competency of staff and capacity levels within the community, poor exchange of information including clinical handover, care plans and client goals, and little use of common communication tools. The purpose of this first part of the study is to examine the methods and tools used for implementing a seamless transition of care in rehabilitation pathways in Sør-Rogaland. We are investigating all methods of information exchange and coordination but focus on digital communication and technological tools in use. The study methods are participant observation and analysis of the patients? electronical medical records. The first year of the project has been used to prepare and apply for ethical approval. All approvals are now given, and we start recruiting patients from this fall. We are also in the beginning of establishing a user panel. Later in this project, we are going to investigate how the involved persons experience the care coordination, and how healthcare staff experience the use of coordination tools, conducting interviews.

Bakgrunnen for prosjektet er satsingen på en interkommunal ambulant rehabiliteringstjeneste (IKART) for voksne mellom 18 og 67 år som trenger intensiv og kompetent rehabilitering i hjemkommunen etter utskriving fra sykehus. Dette er helt ny bred tverrfaglig modell som avhenger av at mange aktører på ulike helsetjenestenivå og fra ulike kommuner over et større geografisk område samarbeider tett. Gode overganger i forløpet påvirker utfallet av rehabiliteringen til den enkelte og er derfor av stor betydning. Samtidig må rehabiliteringstilbudene drives innen forsvarlige rammer med tanke på ressursbruk. Hensikten med denne studien er derfor å undersøke metoder for samhandling, hvordan påvirker teknologi og andre tradisjonelle samhandlingsverktøy kvaliteten på tilbudet og overgangene, for eksempel utskrivning fra sykehus til rehabiliteringsenhet i hjemkommunen og samhandling med fastlege. Studien vil benytte kvalitative metoder som casestudie, deltakende observasjoner og intervjuer for å finne svar på forskningsspørsmålene.
