Modeling the Engineered Heart: Coupled Electromechanical Simulations of Cardiac Regenerative Medicine in the Infarct Injured Ventricle

The loss of functional heart tissue resulting from serious myocardial infarctions is considered a largely intractable problem with a substantial health and economic burden in the industrialized world. However, new hope is emerging in the field of regener ative medicine, where transplanted stem cells are used in an attempt to replace lost myocytes. We wish to investigate this important field theoretically with computational biomedicine, building ventricular simulations of the infarct injured heart and pot ential regenerative medicine treatments using advanced software systems and measured biological data. The aim of the proposed post-doctoral fellowship project is to use a biologically accurate model of an infarct injured left ventricle to simulate the ty pes of treatments utilizing cells and biomaterials currently under investigation in regenerative medicine. A left ventricular model has been constructed based on in vivo measurements of an ovine heart with an induced antero-apical infarct provided by int ernational collaborators, and include geometry, myofiber angles, and strains at end-diastole and end-systole. A mathematical simulation coupling the electrical / mechanical behavior of this left ventricle will be fine tuned to match this data, providing a validated system useful for studying important stress fields and testing hypotheses of electromechanical dysfunction. This model will then be modified to reflect structural changes associated with tissue engineering applications, giving us the ability to theoretically evaluate novel tissue engineering ideas. Our simulations will provide insight into previously obtained clinical and preclinical data and provide results that researchers and clinicians can use in the development of new treatments and the interpretation of in vivo metrics.
