BOLD MINDS aspires to enhance the understanding of vulnerability factors underlying the emergence of borderline personality disorder (BPD), with an ultimate aim of increasing the potential for young individuals to receive early, evidence-based help when their mental health is at risk. BOLD MINDS will achieve this by investigating whether disruptions in two aspects of autobiographical memory (AM), narrative identity (NI) and involuntary AM (IAM), are associated with BPD features and problem behaviours in adolescents concurrently and over time. In doing so, BOLD MINDS will strengthen the understanding of mechanisms underlying the emergence of BPD in adolescence and lay a groundwork for developing evidence-based early interventions for BPD. While currently limited, the study of AM in this field is essential for two reasons. Firstly, because the aspects of AM under investigation, NI and IAM, are known to be involved in identity formation and emotion regulation which represent two key psychological processes that are disrupted in adults with BPD and are linked to problem behaviours such as self-harm and suicidality. Secondly, because promising research is emerging on the use of AM targets in the treatment of emotional disorder in adults, and provides a strong model for future treatment development with vulnerable adolescents. In addition, this timely research program aligns with calls from international organizations to prioritize early interventions to promote adolescent mental health. BOLD MINDS will adopt and adapt state-of-the-art ecological momentary assessment and analytical techniques and work at the crossroads of clinical and cognitive psychological perspectives, allowing for a multidimensional approach to understanding BPD vulnerability. Training-through-research and formalized training of the experienced researcher will prepare her for a permanent academic position and the founding of her own research lab specializing in AM and early interventions for BPD.

Borderline Personality Disorder: Memory and Identity in Adolescence

Bilingual Experiences, Cognitive Ageing and underlying Mechanisms

The goal of the Bilingual Experiences, Cognitive Ageing and underlying Mechanisms (BECAME) project is to examine the functional neuroplastic mechanisms underlying mitigatory effects of bilingual experience on age-related cognitive decline. While evidence is available for structural brain changes supporting bilingualism-induced effects on cognitive aging (CA), functional mechanisms underlying such effects are not yet well understood. BECAME aims to fill this gap by investigating the trajectory of bilingualism-induced functional neuroplasticity across the adult lifespan (ages 18-85), crucially adopting an individual differences approach related to measurable degree of bilingual engagement. Importantly, although such benefits are thought to be rooted early in the lifespan, evidence for the timeframe and conditions of their early emergence is scarce. The choice of including the whole adult lifespan allows us to investigate these mechanisms and their emergence more directly from a lifespan perspective. I combine behavioral testing with brain recordings via electroencephalography (EEG). EEG is widely used in cognitive neuroscience to assess brain activity and the degree of communication between brain regions (functional connectivity) in response to stimuli and at rest. Combining behavioral and EEG modalities will allow me to test bilingualism’s effect on functional connectivity, as well as its consequences for functional efficiency, the minimum required neural activation to successfully perform a task. This will enable a detailed, evidence-based account of the protective effects observed in older-aged bilinguals, and how they are built from earlier life stages. By illuminating the relationship between multilingualism and resulting neurocognitive adaptations, BECAME will enable to capitalize on it as a health initiative, with manifold societal implications and the potential to be translated into actionable policies exploiting multilingualism to ameliorate effects of CA.

WHeelchair Activity Monitoring project (WHAM)

Wheelchair users (WCU) are three to five times more inactive than the general population, leading to a high risk of developing preventable secondary conditions. Although physical activity (PA) guidelines have been specifically developed for WCU, we lack systematic approaches to implement them in practice, as well as valid algorithms to monitor PA change in WCU through sensor-based solutions. Accordingly, the overall scientific aim of the WHeelchair Activity Monitoring project (WHAM) is to advance PA research and practice in WCU by developing valid digital markers for monitoring of and providing feedback on PA, paving the way for implementation and intervention. I will accomplish this through combining the complementary research fields of 1) behavioural intervention studies focusing on increasing PA in WCU (core competence University of British Columbia, UBC; outgoing phase) and 2) sensor-based PA monitoring (core competence Norwegian University of Science and Technology, NTNU; returning phase – consolidated by non-academic placement at Maastricht Instruments, which develop research prototypes including PA monitors). WHAM’s scientific aim is achieved through three objectives: 1) Measure the effect of a pragmatic exercise intervention trial on self-reported physical activity levels and cardio-respiratory fitness, 2) Develop valid algorithms for tracking PA markers in WCU, and 3) Co-create a framework that outlines how PA can be conjointly monitored and provided feedback on in WCU. This fellowship will enable me to become established as an internationally recognized researcher in the field of “physical activity and disability”, while bringing together ongoing national disjointed research activities in this field. Altogether, my overall ambition is to contribute to reducing inequalities in daily-life participation among WCU compared to the rest of the population.

Longitudinal Investigation of Visual and Numerical Perception in Preterm and Full-term Infants

The number of preterm births is on the rise -now more than 1 in 10 babies- and being born preterm is associated with having difficulties in two very important and related cognitive skills that we rely on daily: numerical and visual perception. However, a knowledge gap exists on neural and behavioural associations of numerical and visual perception in full-term and preterm infants. PrePER aims to uncover the underlying neural signatures of the cognitive dysfunctions that preterm infants might display in the first year of their lives and the role of parent-baby interaction to predict preterm infants that are at risk of developing these cognitive difficulties in the second year of their lives. The purpose is to establish criteria to identify infants (especially preterm) at-risk of having cognitive difficulties in numerical and visual perception. Shedding light on normal and impaired cognitive functions in infancy is an essential stepping stone that will pave the way for early diagnostic tools and interventions that would correct the cognitive dysfunctions more efficiently while the brain is still developing. The activities in PrePER will lead to original and innovative outcomes that are beneficial for four parties: 1) Scientific communities will be interested in the results as the ambitious research objectives address questions that have not been asked before. 2) Parents of preterm infants, health-care providers, and policy makers will be interested in the results as the overall aim is to establish criteria to identify infants at-risk of having cognitive-developmental difficulties in the first year of their lives. 3) The researcher will be accelerating her career progression as the training objectives meet her specific needs. 4) The supervisor will be enlarging her research interest and network as the research objectives include a topic within the researcher’s expertise.

AQUASERV – RESEARCH INFRASTRUCTURE SERVICES FOR SUSTAINABLE AQUACULTURE, FISHERIES AND THE BLUE ECONOMY

The overarching objectives of AQUASERV are to bring together, enhance, integrate and customise RI capacities (including facilities, instruments and expertise) and through the provision of transnational access (on-site or remote) and/or virtual access, to significantly further scientific advance and promote and facilitate the implementation of European Common Fisheries Policy, the Farm to Fork Strategy, the Sustainable Blue Economy and the European Green Deal. 

AQUASERV will achieve these objectives by offering scientists from academia and business remote and on-site transnational (TA) and virtual (VA) access to an advanced set of European research infrastructures and its nodes related to research and management of marine and freshwater biological resources, food and biotechnology. These include the European Marine Biological Resource Centre ERIC (EMBRC), the Analysis and Experimentation on Ecosystems ERIC, The Aquaculture for Excellence for Aquaculture in Fish (Aquaexcel), and the Infrastructure for Promoting Metrology in Food and Nutrition (METROFOOD), as well as the International Council for Exploration of the Sea, both a service provider and stakeholder. It also includes the Research Infrastructure for Science and Innovation Policy Studies (RISIS) to bring contributions from the social sciences and to bring results nearer to policy makers ensuring societal impact. 

To enhance, integrate and customise RI capacities, collaboration between partners and new pipelines to meet the objectives of the project will be developed during the first 3 years and will make available  to users.  To ensure these capacities will not be lost, a sustainability work package involving stakeholders will address the project legacy.

IMProving User experience, Long-term sustainability and Services of EU-OPENSCREEN

EU-OPENSCREEN (EU-OS), the distributed European Research Infrastructure for Chemical Biology and early drug discovery, provides world-class services in high-throughput compound screening and medicinal chemistry to users from academia and industry. As one of the largest open-access initiative globally, EU-OPENSCREEN plays an increasingly important role in facilitating early stage drug discovery in Europe. It recently integrated fragment-based drug discovery and chemoproteomics as two new services, but the increasing complexity of modern drug discovery projects requires an even more comprehensive portfolio and integration of emerging technologies. To ensure that EU-OS continues to offer a comprehensive suite of relevant services and support a growing user community to conduct forefront research, IMPULSE will develop, validate and integrate new services in AI/ML, new chemical modalities, and innovative disease-relevant cell models combined with CRISPR-Cas9/RNAi-based genetic screening. IMPULSE will work towards the implementation of common operational and data standards to increase data reproducibility and FAIRness. Bespoke outreach and training activities will raise awareness among key user groups of its new services and advance operational excellence and reproducibility in pre-clinical drug discovery. New models for pre-competitive drug discovery with industry or charities, and growing ERIC member community will reinforce the sustainability and accessibility of EU-OPENSCREEN.

Strengthening resilience and mental wellbeing through the Support4Resilience toolbox for leaders in elderly care

European elderly care experiences labor shortages, lack of qualified workers and a mismatch between capacity and demands, causing stress, burnout, and reduced mental wellbeing among healthcare workers and informal caregivers. Leaders are crucial in promoting supportive working conditions but lack research-based measures to act upon these challenges.

Support4Resilience (S4R) will develop, implement, and evaluate a research-based Toolbox to support healthcare leaders in improving healthcare workers’ and informal caregivers’ resilience and mental wellbeing in elderly care. S4R will identify resilience and mental wellbeing factors among healthcare workers and informal caregivers; explore their perspectives and needs; develop new theory on the relationship between individual and organizational resilience, and mental wellbeing; and develop recommendations and cost-effective interventions. The Toolbox with tailormade resources for policy and practical use will be available through an open access S4R Resource Bank.

The S4R Toolbox includes three main tools: 1) Mapping and identification, 2) Reflection and education, and 3) Reorganization. Measures are initiated at the organizational level and effects on mental wellbeing and resilience transpire at the frontline of elderly care. The Toolbox covers different situational contexts, types of healthcare providers and health system levels. Elderly care settings in six European countries will implement and evaluate the Toolbox through a process evaluation, effectiveness evaluation, and cost-effectiveness evaluation. 

S4R provides policymakers, decisions-makers and leaders with solutions for taking action to solve specific risks for healthcare workers’ and informal caregivers’ resilience and mental wellbeing. Thus, pertinent to the work programme, S4R will support the development of resilient health systems in European elderly care through improved leadership capabilities, governance structures, and adaptive capacities.

NEXT GENERATION IMAGING FOR REAL-TIME DOSE VERIFICATION ENABLING ADAPTIVE PROTON THERAPY

The NOVO project will address a fundamental issue in proton radiotherapy (PT) of cancer, namely the lack of real-time dose verification (RDV) that significantly limits the potential of PT due to uncertainties in the precise delivery of the prescribed dose. In the NOVO project, we will develop the first proof-of-concept of an RDV technology adaptable to any PT treatment and bring the concept to TRL 4 by also considering integration into the PT clinical workflow. The RDV technology will be pivotal for enabling the next generation of adaptive PT therapy, i.e., dose-guided real-time adaptive therapy. Real-time measurement of the dose to targeted and non-targeted tissues will be the key to truly empower patient-centered, personalized cancer treatment, important to improve curation and quality-of-life post-treatment. As a high-risk/high-gain project, NOVO aims at a first-of-its-kind RDV technology through the unique synergy between (1) cutting-edge, inexpensive organic scintillator technology that can detect secondary radiation during treatment for non-invasive measurements, (2) novel and fast image reconstruction, artificial intelligence (AI)-accelerated models and AI-enhanced image reconstruction to allow simultaneous detection of multiple radiation species and tissue compositional analysis, (3) tumor-tracking and imaging of tissue radio-sensitivity based on oxygen levels, (4) intelligent automation of decision-making schemes for real-time dose-guided adaptive therapy. The project will essentially open a new era in PT that enables curing more cancer patients and reducing toxicity from treatment. The NOVO consortium covers the entire value chain of RDV development (technology providers, theory and modelling, technology integration and PT clinics) and optimally engages nuclear, medical, and high-energy physicists, chemists, mathematicians, computer scientists, oncologists, biologists, as well as European PT centers. 


Development of innovative proton and neutron therapies with high cancer specificity by 'hijacking' the intracellular chemistry of haem biosynthesis.

Deep-lying cancers, such as glioblastoma multiforme (GBM) brain cancer, are very difficult to access and incurable by the current standards of care. The fluorescence of photosensitive molecules (PSs) can precisely guide surgical resection of GBM. Nevertheless, photomedical treatments like photodynamic therapy (PDT) show limited efficacies due to the low penetration of light into tissue. Proton radiotherapy can reach deep-lying disease through therapeutic energy deposition in a spatially confined region called Bragg peak. Current neutron-based therapies, like Boron Neutron Capture Therapy (BNCT), have sufficient penetration depth but lack cancer specificity.
In NuCapCure, we propose to 'hijack' intracellular biosynthetic pathways and use them to produce custom-made drugs that will facilitate two genuinely new cancer treatments: i) "NuCapCure Proton", combining proton radiotherapy, protondynamic therapy through PS proton activation, and proton capture therapy (BPCT) through three produced alpha particles; and ii) "NuCapCure neutron", combining GBM specific BNCT and PS neutron activation as demonstrated in our FET-open project FRINGE.
The main scientific breakthroughs of NuCapCure will be to demonstrate the custom production of novel compounds through native intracellular biosynthesis but also to establish experimental proof-of-principle, both in vitro and in vivo, of the proposed two novel therapies. Our highly interdisciplinary project team comprises world-renowned experts from a unique combination of disciplines: proton and neutron physics, synthetic chemistry, photochemistry, photobiology, radiobiology, medical physics, nuclear reactor and particle accelerator technology and radio-oncology to lay the foundations for novel, targeted and curative therapies against currently fatal cancer indications like GBM.

Despites dental pulp capping materials should meet the certain requirements, the available materials have one or more shortcomings—poor biocompatibility and rheological properties, fail to maintain pulp vitality and to simulate reparative dentine formation and expensive etc. Mineral trioxide aggregate (MTA) is widely used material because of its beneficial properties. However, its composition bismuth oxide, which is suspectable to toxicity, has an impact on rheological properties. This emphasizes the necessity of looking for inexpensive alternatives to materials that meet the specifications. This project's goal and novelty are to investigate the potential processing of the dental pulp capping cement, which is made of strontium silicate (SrSiO3), strontium fluoride (SrF2), piezoelectric material (PZM) and tricalcium silicate (Ca3O5Si) and its rheological properties and hemocompatibility. Because SrSiO3, SrF2, PZM and Ca3O5Si have the promising properties. This project's work packages are split into four categories based on the interdisciplinary approach since it involves the synthesis and characterization of PZM, the processing and characterization of cement, the identification of rheological parameters utilizing ISO standardizations, and hemocompatibility, and acquiring new transferrable skills. Additionally, the interactions among the selected compounds will be determined. The scientific impact of this project is comprehending the physical properties and the immune system's response to evaluate the cement's suitability as a replacement for existing materials, as well as resulting in further uses in dentistry and medicine. The societal impact is providing millions of patients in Europe with predictable and outstanding treatment outcomes each year to maintain pulp vitality and health. Eventually, it will ease the financial load on millions of patients who need dental care, will be avoiding inflated medical expenses and misuse of the patent (economic impact).

Digitalisering refererer til endringsprosesser der digitale løsninger erstatter det som før ble gjort manuelt eller ved hjelp av analog informasjon. En av faktorene som avgjør om disse endringsprosessene vil lykkes i helsesektoren, er hvor godt de ansatte settes i stand til å bruke de nye digitale systemene og verktøyene i sitt arbeid.   

Det pekes på at den store andelen deltidsansatte i små stillinger i den kommunale helse- og omsorgstjenesten kan bidra til å bremse digital omstillingsevne i denne sektoren. Ansatte i små stillinger har dårligere muligheter til å sette seg inn i nye systemer og rutiner på grunn av tidsnød, høyt arbeidspress og lengre tid mellom hver gang de er på jobb. Det er derfor behov for å undersøke hvilke krav til ansattes ferdigheter og omstillingsevne digitaliseringen medfører, og hvordan digitaliseringen påvirker arbeidsflyt og samarbeid, samt hvordan det kan legges til rette for læringsprosesser som inkluderer og motiverer alle ansatte. Det er også nødvendig å se på hvordan helsepersonells muligheter og motivasjon til å omstille seg virker inn på tjenestekvalitet, slik dette gir seg utslag i brukeres helse, tjenestebruk, brukerinvolvering og opplevelse av tjenestene.  

I forskningsprosjektet EHCOM skal vi undersøke disse forholdene. EHCOM består av en tverrfaglig forskergruppe fra SINTEF Digital og NTNU, i tett samarbeid med ledere, ansatte og innbyggere fra de fire kommunene Malvik, Levanger, Hitra og Verdal. Prosjektet kombinerer perspektiver og metoder fra samfunnsvitenskap, helseøkonomi og helsetjenesteforskning.

EHCOM responds to the call for proposal related to e-health by addressing the primary health care sector's readiness for digital transformation in terms of competence and organization of the workforce. Digitization corresponds to the process of changing from analogue to digital form and digitization is the generic term for the digital transformation of society. Health care staff's competency in handling digital systems and adopting digitization in practice play a key role in achieving efficient and high-quality services. Studies do however indicate that the health care sector has been less successful in exploiting the potential of digital technology, and that especially part-time workers in municipal health care miss out on learning opportunities and competency development due to high work pressure and lack of time. Thinking through the implications of technology for skills demand, work process alterations and how to enable up-skilling is required. A health care sector taking full advantage of the competency of all its workers during radical digital transformations will advance the quality and the operational efficiency of its services.  

EHCOM consists of a multidisciplinary team with researchers from SINTEF, NTNU, and municipal leaders and practitioners from four municipalities. The project combines perspectives and approaches from the disciplines of social science, health services research and human-technology interaction. The proposed study is based on a long-standing collaborative relationship between the research team and the municipalities, and the project will involve close collaboration, common activities, and mutual learning between project partners and relevant stakeholders. The four municipalities will fully finance their own activities, while NTNU will finance major part of their commitments in the project.

Ensuring sustainable e-health transformation in primary health care through competent full-time workers

Langvarige smerter gjør mange personer i Norge ut av stand til å jobbe. For de som rammes, kan det være utfordrende å komme tilbake til arbeidslivet. Deltakelse i arbeidslivet er bra for fysisk og psykisk helse. Personer med psykiske lidelser kommer lettere tilbake i arbeid med individuell jobbstøtte. Hva med de som har smerter?

Hva vil vi finne ut av? 
Vi vil finne ut om individuelle støttetiltak med arbeidsplassering kan hjelpe personer som er utenfor arbeidslivet på grunn av smerter, til å komme tilbake i arbeid, og om dette bidrar til bedre helse og livskvalitet.
Vi har utført en forundersøkelse i England, og tiltaket ble tatt godt imot av de med smerter som var utenfor arbeid. Det er behov for en fullskala studie i Norge for å sammenligne effekten av dette tiltaket mot de eksisterende tilbudene for å hjelpe disse personene til å komme tilbake i arbeid.

Hvordan vil vi gjøre dette? 
Vi vil rekruttere personer som har vært uten jobb i minst én måned, og som har hatt smerter som påvirker arbeidsevne i mer enn tre måneder. Én av fire vil tilfeldig utvelges til tiltaket, mens resterende følger eksisterende tilbud. 
En saksbehandler vurderer de utvalgte deltakernes arbeidsevne, hva som hindrer dem i å jobbe, og gjennom dialog blir deltaker og saksbehandler enige om en plan for å komme tilbake i arbeid. Saksbehandleren finner en passende seks ukers arbeidsplassering og vil underveis støtte deltaker og arbeidsgiver, koordinerer involverte parter, og henvise til arbeidsrelatert helsehjelp ved behov. Tre, seks, og 12 måneder etter oppstart av tiltaket vil vi sende et spørreskjema til deltakerne i studien der vi kartlegger om de er i jobb, hvor mye smerter de har, og hvordan de oppfatter egen livskvalitet. Dette vil gi oss svar på om arbeidsplassering med individuelle støttetiltak er verdt å tilby folk som ikke klarer å jobbe på grunn av smerter.   

ReISE er et samarbeid med Manpower, OsloMet, University of Warwick og Rådet for muskelskjeletthelse.

Increasing work participation for people with persistent pain improves their health, quality of life, well-being, and reduces poverty. However, it is not clear how best to help unemployed people who have persistent pain. While existing employment services in Norway are flexible, they are not usually coordinated with work-focused healthcare. Supported employment is effective for helping people with mental health challenges and there is interest in adapting this for people living with persistent pain. However, while mental health challenges often accompany persistent pain, people with pain may also need physical and organisational workplace accommodations. We have developed an intervention with matched supportive work placements and coordinated work-focused healthcare. A trained case manager helps a person identify obstacles to working and collaboratively agree a return to work plan. The case manager matches the person to a placement, acts as a bridge between systems, provides support, and makes referrals for work-focused healthcare where necessary. In a UK feasibility study, we confirmed this intervention was feasible and valued by those involved. Around 20% obtained paid employment within the short six-month follow-up period. This is promising considering that after two years (without intervention) the probability of return to work in the foreseeable future is thought to be close to zero in this population. A full-scale trial is needed to explore the effectiveness and cost-effectiveness conclusively. With Manpower, a leading staffing provider (for placements), a healthcare council, OsloMet, and University of Warwick, UK, we will adapt the intervention for use in Norway, pilot its delivery, and do a full-scale trial to examine whether the intervention is cost-effective for improving return to work and health-related quality of life. If successful, the intervention will have the potential to help a large group of people who are unemployed and have persistent pain.

Returning people with persistent pain to work using Individual Supported Employment placements (ReISE)

Livmorhalskreft kan forebygges ved at kvinner rutinemessig tar celleprøve fra livmorhalsen. Målet i VisMeg er å utvikle digitale tjenester til deltakere i screeningsprogrammet og helsepersonell som er involvert. Den medisinske utviklingen fører stadig til endringer i prøvetyper og screeningintervaller. Dette gjør at deltakere, men også leger, kan bli usikre på hva prøvesvarene betyr og hvordan unormale prøver skal følges opp. 

Kreftregisterets rutiner for utlevering av data er i dag manuelle og tidkrevende prosesser. Både deltakere og helsepersonell forventer enkle digitale løsninger for innsyn i prøvesvar og prøvehistorikk. Men registerdata er ikke umiddelbart forståelige for deltakere, og kan utløse unødvendig bekymring. Den største forskningsutfordringen er å presentere registerdata og prøvehistorikk til ulike brukergrupper på en presis og forståelig måte.

Prosjektet skal utvikle modeller, metoder, verktøy og prototyper for å gjøre dette mulig. Ved å tilby beslutningsstøtte skal det bli enklere å ta prøver ved riktige intervaller. Videre skal screeningdeltakerne få innsyn i hvordan deres helsedata lagres, hvem som har tilgang, og hvordan informasjonen sikres. Dette håper forskerne vil stimulere til økt tillit og dermed økt deling av helsedata og at registerdataene utnyttes mer effektivt til beste både for den enkelte kvinne og for helsetjenesten.

VisMeg er et samarbeid mellom Kreftregisteret, SINTEF Digital og Kreftforeningen. SINTEF bidrar med spisskompetanse på modeller, visuelle språk og brukersentrert utvikling. Kreftforeningen vil bidra til innsiktsarbeid og evaluering gjennom sitt omfattende nettverk og brukerpaneler.

Prosjektet vil bidra til redusert tids- og ressursbruk hos Kreftregisteret rundt utlevering av data. En optimal screeningdeltakelse vil også lede til mer effektiv ressursutnyttelse i helsevesenet og gagne folkehelsen. Prosjektresultater vil gjøres offentlig tilgjengelig slik at flere kan benytte resultatene.

Livmorhalsprogrammet (LP) i Kreftregisteret har ansvaret for å forvalte og videreutvikle screening mot livmorhalskreft, og har helsedata om nesten 2 millioner kvinner. Hvert år sender LP påminnelse til ca 450 000 kvinner om at det er tid for ny prøve. Mye endringer med nye tester og nye retningslinjer i LP fører til at fastleger og pasienter blir usikre på hva prøvesvarene betyr, og hvordan kvinner med unormal prøver skal følges opp. 

Kvinner og deres behandlende leger har en lovfestet rett til innsyn i helsedata i LP. Per i dag finnes ingen rask tilgang til data i LP. Rutiner for innsyn er svært ressurskrevende. Innsyn går via fastlege og kan ta opp mot 30 dager. Manglende oversikt over tidligere prøver kan føre til unødvendig prøvetaking, og til feil utredning.

Hovedmålet er å utvikle en digital tjeneste, VisMeg, for innsyn og veiledning knyttet til registerdata på en forståelig måte. Prosjektet skal utvikle modeller, metoder, verktøy og prototyper for visuell presentasjon av heterogene helsedata. Prosjektet skal også tilby beslutningsstøtte og veiledning basert på data og prøvehistorikk, samt gi innsyn i dataforvaltningen for økt tillit. Prosjektet tar utgangspunkt i det nasjonale screeningprogrammet mot livmorhalskreft, og vil ha overføringsverdi til andre screeningprogram og helseregistre.

SINTEF, LP og Kreftforeningen vil i prosjektet innhente brukerinnsikt, kartlegge behov og barrierer, utvikle en semantisk modell og definere et grafisk språk for kommunikasjon av medisinske data, utvikle prototyper, og evaluere dem sammen med brukere.

En slik digital tjeneste vil bidra til å øke dekningsgraden, optimalisere forebygging av livmorhalskreft, redusere unødvendig prøvetaking og dermed spare store ressurser for innbyggere, Kreftregisteret og samfunnet. 

Flere pasientorganisasjoner og spesialistorganisasjoner vil være viktige støttespillere, og vil inviteres som studieobjekter samt ha en rådgivende funksjon for forskningen, men også realiseringsarbeidet.

VisMeg: Hvordan kommunisere medisinske registerdata til innbyggere på en forståelig måte?

Det er programfestet at de statlige spesialisthelsetjenestene ved sykehusene og kommunehelsetjenestene skal samvirke for å få en mer effektiv pasientflyt. Men det har, over tid, blitt avdekket et gap i kompetansegrunnlagene mellom stat og kommune og mellom helsetjenestene som gjør samhandling vanskelig, for ikke å si umulig i noen sammenhenger. Dette kan føre til alvorlige konsekvenser for pasientsikkerhet og behandling. Prosjektet skal bidra til at kompetansegapet lukkes. 

Basert på kompetansemodellen KlinObsKommune, skal prosjektet designe og implementere simuleringsscenarioer for å trene intra-kommunal samhandling og samhandling mellom kommunale helsetjenester og spesialisthelsetjenester. Simuleringene skal bruke en blanding av fysiske og virtuelle elementer satt sammen for å stimulere samhandlingsferdigheter og optimalisere læring.

Prosjektet skal bruke vitenskapelige metoder for å forstå hvilke samhandlingshendelser som er mest prekære og som man får mest igjen for å trene på. Simuleringsbasert trening skal så utvikles for disse hendelsene, med sikte på å lage gjenbrukbare vignetter som kan settes sammen til større samhandlingsscenarioer. Scenarioene kan realiseres i forskjellige mixed-reality settinger, fra diskusjonsøvelser til ulike former for VR-støttet trening. Prosjektet er i gang med å identifisere en første slik vignett basert på grunnleggende samhandling i KlinObsKommune-rammen. 

Prosjektet skal jobbe i samspill med andre eksisterende aktiviteter som iverksetter deler av KlinObsKommune-modellen. Prosjektertes deltakere har god oversikt over disse ulike aktivitetene og vil jobbe aktivt for å innlemme treningen i opplærings- og arbeidsprosesser, slik at strukturelle barrierer ikke forhindrer læring.

Prosjektet skal innlemme bruken av digitale støtteverktøy i treningen og skal opplyse initiativer som jobber med kartleggingen og utviklingen av slike verktøy om prosjektets funn utfra et treningsperspektiv.

Close the Gap skal adressere kompetanse- og strukturgapet i kommunale og statlige helsetjenester som hindrer effektiv og forsvarlig samhandling. Basert på kompetansemodellen KlinObsKommune, skal prosjektet implementere simuleringer for å trene intra-kommunal samhandling og samhandling mellom kommunale og spesialisthelsetjenester. Simuleringene skal brukes i de ulike helsetjenestene og i helsefaglig utdanning. Simuleringene skal bruke en blanding av fysiske og virtuelle elementer satt sammen for å stimulere samhandlingsferdigheter og optimalisere læring.

Prosjektet er meget godt forankret i tidligere initiativer og i de videre planene for disse. Sentrale personer som har kartlagt utfordringene i helsetjenestene og som står bak kompetansemodellen er med i prosjektet, og fire kommuner som skal implementere modellen gjennom utviklingssentrene er med i prosjektet. Prosjektet har også med miljøer som har utviklet simuleringer for grunnferdighetene i kompetansemodellen. Prosjektet skal fokusere på beslutningstaking og samhandling over grunnferdighetene. 

De sentrale FoU-utfordringene er å forstå (a) hvordan grunnferdigheter skal aktiveres i hendelser slik at de stimulerer beslutningstaking og samhandling (b) hvordan grunnferdigheter, beslutningstakingsferdigheter og samhandlingsferdigheter skal spille sammen i hendelser og simuleringsteknologi for å optimalisere læring og (c) hvordan ferdighetsbygging best kan måles i simuleringene på en relevant måte, slik at man kan dokumentere læringseffekt og tilpasse treningen over tid.

Close the Gap er plassert svært godt i tid. Utredninger og kartlegginger har skapt en bevissthet rundt prekære samhandlingsutfordringer, samtidig som det nå finnes overordnete modeller for å gjøre ting bedre. Prosjektet imøtekommer dermed en nåværende interesse og entusiasme for å iverksette tiltak.

Simuleringsbasert trening for samhandling i og mellom helsetjenestene

Langvarig sykefravær har store konsekvenser både for den sykmeldte og for samfunnet. Den vanligste årsaken til sykefravær er muskel- og skjelettplager. Til tross for betydelig forskning vet vi lite om effektive tiltak, og det mangler kunnskap hvem som trenger hvilke tiltak. Det er heller ikke bare et spørsmål om hvem som vil ha nytte av hva, men også et spørsmål om når de trenger hvilke tiltak. Målet med dette prosjektet er å utvikle og evaluere et beslutningsstøttesystem (SmaRTWork) som kan muliggjøre 1) tidlig identifikasjon av personer som står i fare for langvarig sykefravær på grunn av muskel- og skjelettplager, og 2) gi persontilpassede anbefalinger om tiltak for bærekraftig retur til arbeid. Beslutningsstøttesystemet vil bruke «case-based reasoning» (CBR). Hovedprinsippet til CBR er å bruke kunnskap om tidligere suksessfulle «caser» for å foreslå tiltak for nye og lignende «caser». Personer som har vært sykmeldt i 4 uker på grunn av muskel- og skjelettplager vil bli invitert til å bruke SmaRTWork. De som samtykker til deltakelse vil få tilgang til SmaRTWork, som er web-basert og kan benyttes på pc/nettbrett/telefon. SmaRTWork er et brukersentrert system og det er opp til den sykmeldte å velge om han/hun ønsker å gjennomføre de individtilpassede tiltakene som foreslås. For å øke aksept, tiltro og sjansen for at systemet vil bli brukt så vil vi involvere brukere og andre nøkkelpersoner i utviklingen og evalueringen av SmaRTWork, og brukere vil også inviteres med i prosjektets rådgivende styre. Hvorvidt SmaRTWork har effekt på tilbakeføring til arbeid vil vi evaluere gjennom en randomisert kontrollert studie. I tillegg vil vi gjennomføre en prosess-evaluering for å vurdere hvilke faktorer som hemmer og fremmer implementering, og kvalitative studier for å undersøke hvordan de sykmeldte og andre nøkkelpersoner opplever å bruke systemet.

Musculoskeletal disorders are the most common reason for sickness absence. Despite extensive research on return to work interventions the results are inconsistent and there is a lack of evidence in regards to who needs what measures. Furthermore, it is not only a question about who needs what; it is also a question about when. The aim of this project is to develop and evaluate an intelligent decision support system that i) enables early identification of individuals at risk of long-term sick leave due to musculoskeletal disorders, and ii) provides personalised recommendations to facilitate sustainable return to work. The SmaRTWork tool will utilize state-of-the-art artificial intelligence (AI) methodologies for the early risk prediction and subsequent tailoring of a personalised intervention to facilitate sustainable return to work. The core foundation for the decision support system will be case-based reasoning (CBR). The main principle of CBR is to reuse knowledge from previous successful cases to suggest solutions for new and similar cases. Workers who have been sick listed for 4 weeks due to musculoskeletal disorders will be invited to use The SmaRTWork tool. After consent they will receive credentials to access app-based SmaRTWork tool. The effect of the SmaRTWork tool will be evaluated in a randomized controlled trial. The main outcome will be time to sustainable return to work (i.e. one month back at work without relapse) based on registry data. There will be a process evaluation that will assess facilitators and barriers for implementation, and qualitative studies to explore how the sick-listed workers and other relevant stakeholders experience using the system.
