KEYSTONE An open platform for integrationg digital applications for senior citizens living at home

Avoiding institutionalisation for European senior citizens who face challenges with cognitive impairment, frailty and multiple chronic health conditions is an explicit political goal; the elderly are now expected to live at home for as long as possible. Senior citizens with temporary or long-term illness who are living at home, and their next of kin and informal caregivers, need support for tasks related to care, treatment and social inclusion of the elderly. Addressing the above challenges through technology, i.e., ensuring trustworthy digital services supporting self- and informal care at home and social inclusion of the elderly in their local communities; require open platforms with functionalities that can be generically reused in multiple context to support various types of application and services. The platform and the piloted applications will assist the elderly in their everyday lives at home -not only with treatment and medical monitoring; but equally with aligning such activities with social participation. For health service providers and SMEs offering digital tools for smart and healthy living at home KEYSTONE is proposed as the open platform to harness technologies that support self-management of illness and everyday health related activity coordination for European citizens. Expected results are: • An open platform build on state of the art as of 2018, open for the integration of different technologies • Integrated solutions for coordinating social and care activity, treatment and monitoring results, piloted in at least 5 countries. Measures for socio-economic impact and QoL outcomes included. • A generic model for secure and safe implementation and organisation of person-centred coordination and monitoring technologies • Secure solutions for harvesting data across the integrated solutions • Synthesis of evaluations and knowledge transfer/local adaptions across participating national initiatives • Uptake of the platform in the ecosystem.
