Demonstrating the impact and sustainability of digital solutions for personalised and outcome-based integrated CARE in rural areas

CareMatrix is our response to the call forSC1-DTH-11-2019 Large Scale pilots of personalised & outcome based integrated care. Specifically, CareMatrix will contribute to the development and deployment of digital technologies that support complex and shifting care trajectories in home and community care for the frail elderly (including older people with functional impairments) and their informal carers. We will be targeting Research and Innovation action requesting for a large budget of approx. 9 million. The summary of H2020 project proposal is enclosed in the attachment.
