Application preparation for Eurostars project proposal FLEXCARE to be submitted within the Eurostars 2019 cut off, September 12 2019.

FLEXCARE provides a comprehensive multilingual solution for home care delivery that brings together patients, formal and informal caregivers with the goal of allowing patients to remain longer at home and feel independent. FLEXCARE is composed by: - A patient & informal caregiver app (CCARE) >> situation awareness, dynamic escalation - An IoT based eHealth platform (Tellu) >> sensor connectivity - A professional portal (HIB) for caregiver management >> efficiently scheduling, routing and managing the process of assignment of caretakers to patients’ home - A professional mobile app (HIB) >> scheduling and all the patient information in mobility. FLEXCARE will facilitate self-management of daily care by patients exploiting information gathered from sensors and devices that will enable to continuously monitor the activity behaviours of patients in their home. This data together with patients’ self-reports will allow increasing awareness on the current situation of patients and, in case of incidents occur; conduct a dynamic escalation procedure, overviewing the availability of informal caregivers involved in the care of a patient, to solve it. FLEXCARE will also offer the provision of on-demand service, through an interface to various service providers (e.g. unscheduled and promptly support from professionals in FLEXCARE, taxi, cleaning).
